Epidemic Status Of MS In Elderly And Its Relationship With CVD/Stroke Based On Clinical And Experimental Study

To study the epidemic status of metabolic syndrome (MS) in Chinese elderly, 1807 subjects aged from 40 to 95 were investigated and prevalences of abnormal metabolic indices, MS and CVD or stroke were analyzed by work definitions of IDF, WHO and CDS respectively. Prevalences of MS were 34.37%, 39.01% and 38.90% by woke definitions of IDF, WHO and CDS respectively. The consistency of three work definitions was 73.6%. The prevalences of CVD and stroke were high in MS population. Then we compared the abnormal metabolic indices between two groups with or without CVD/stroke. The correlation between metabolic indices and prevalence of CVD/stroke was analyzed. Waistline, WHR, ratio of waistline to body height, FBG, PBG and SBP of group with CVD/stroke were higher than those without CVD/stroke (p<0.05). Prevalence of CVD/stroke elevated accompanied by the increasing number of abnormal metabolic indices. Male, aging and MS were correlated with CVD/stroke significantly. Hyperglycemia and dyslipdemia were important to CVD and hypertension was important to stroke. To study the correlation between different obesity indices and CVD/stroke, we devided 1807 subjects by the level of BMI, waistline, WHR and ratio of waistline to body height. Then prevalences of CVD/stroke were calculated in different groups. The results indicatedthat prevelance of CVD/stroke elevated accompanied by obesity aggravating. Ratio of waistline to body height was the best index which represent abnormal metabolic status. When ratio of waistline to body height was beyond 0.55, there was always that other two abnormal metabolic indices appeared simultaneously and odds ratio of CVD increased significantly. So we concluded that ratio of waistline to body height was a good index which indicate obesity.High level of plasma free fatty acids was related to not only MS but also CVD/stroke. In our past study, PA induced injury of endothelial cells. AMPK was an enzyme regulating energy metabolism. Activated AMPK promote fatty acids oxidation and improve IR. To study the protective effect of AMPK on the HUVEC damage induced by PA, HUVEC were cultured in DMEM contained PA, AICAR, metformin or pioglitazone, respectively for 24h to 72h. Survival rates of HUVEC were determined by MTT. Western blot were performed to detect the expression of phosphote-AMPK. The results were that survival rates of PA decreased gradually and AICAR, metformin and pioglitazone can extenuated the injury induced by PA accompanied by increasing expression of phosphote-AMPK. In next study we found that PA induced increasing expressions of mRNA of MCP-1 and protein of ICAM-1, activating AMPK could decrease expression induced by PA. Then we found that activating AMPK could prevent the activation of NFkB induced by PA. So we supposed that activating AMPK downregulated the increasing expression of MCP-1 and ICAM-1 by the pathway of NFkB/IkB.
