Study Of Knowledge Reconfiguration For Clinical Pathway Implementation

The reform of medical insurance on case payment will be implemented inour country, and it will urge the introduction of clinical pathway managementmode to many hospitals. But there are some problems in its implementation. Mostof them are related with the lack of the direction of the systemic knowledgereconfiguration methodology.This thesis makes a deep research on the knowledge and knowledgereconfiguration in the clinical pathway implementation, sets up hospitalknowledge model, and theory and methodology architecture for clinical pathwayimplementation. The effective and systematic guiding tools are provided for theclinical pathway implementation.The thesis analyzes the route of the clinical pathway implementation, pointsout the characteristics of clinical pathway and its demand on the knowledge. Incontrast to the current knowledge condition of hospital, the hospital's demand onthe knowledge reconfiguration is discussed and the imminence of research on theknowledge reconfiguration for clinical pathway implementation is pointed out. The content, layers and characteristics of the hospital knowledge are studied,and the general concept model and mathematics layer model are set up. On thebasis of them, the knowledge architecture for the clinical implementation is set up.In the virtue of the model, this thesis firstly describes the content, layer andcharacteristics of the knowledge in clinical pathway implementation, builds upknowledge reconfiguration theory and related models for clinical pathwayimplementation, and sorts the knowledge in clinical implementation according tothe knowledge reconfiguration architecture. Based on that research, the thesis sets up the general task model of knowledgereconfiguration for clinical implementation. It is the center for the building of thereconfiguration task table model, task implementation route model of theknowledge reconfiguration and nine work tables, knowledge reconfigurationprocess model and reconfiguration method model. The CommonKADSmethodology of knowledge engineering is used for reference in thereconfiguration table model described in UML language, task implementationroute model of the knowledge reconfiguration and nine work tables. Theknowledge reconfiguration method architecture for clinical pathwayimplementation including concept model, process model, implementation toolsand realization model are formed. The powerful knowledge reconfiguration toolsfor successful clinical pathway are provided. At last, the thesis applies the game theory to analyze the effectiveness,influence factor and impetus mechanism of knowledge reconfiguration on thecomplicated layer, and applies knowledge reconfiguration theory andmethodology in the clinical pathway implementation in a hospital.
