Background:As the most common crystal induced arthritis, gout has a rising prevalence nowadays, and now it has become a common chronic disease. When talking about chronic disease, outpatient clinic is an important way for management. Since the lack of general practitioner, the outpatient has to refer to 3 armor hospitals for help, which makes patient education more difficult as a result of enormous patient quantity. At the same time, since our clinic has a large number of chronic gout patient, we are trying to find out what makes gouty arthritis chronic.Objective:1. To investigate the gout patients’management of the status quo.2. To find out how to make patient education more effective.3. To design a web application for the management of gout patients.4. To explore if IL-17 has something to do with chronic gouty arthritis.Method1. A structured survey was carried out by telephone and wechat interview in 130 patients recruited from Gout Clinic at Peking Union Medical College Hospital. The management adherence, disease control and the reason for lack of adherence were investigated by semi-quantitative interview.2. A web applicationbasedonwechatopenplatform was designed for management of gout patient.3. Using 56 serum samples and 4 blood samples from 56 patients recruited from Gout Clinic at Peking Union Medical College Hospital, an experiment was design to find out whether there is difference of IL-17 secretion between patients with and without tophus.Result1. The proportion of dietary and medication adherence is 49.2% and 36.9%. The reason for lack of medication adherence are feeling of good control, no willing to take medication, worry about side effect, no medication prescription and acute gout attack after medication, et al.2. More test of serum uric acid is related to good medication adherence.3. IL-17 showed a rising level of mRNA expression among gout patients, while no serum IL-17 was detected among both gout patients and healthy people.Conclusion1. With more patients coming from out of town and more patients with chronic gout, we need to focus on the point which has been ignored when carrying out.2. Management of gout patient need the involvement of patients.3. IL-17 may have a rising level of expression among gout patients, but it need more investigation.
