Study On The Regularity Of Syndrome Differentiation Of Irritable Bowel Syndrome Based On Chinese Journal Of Traditional Chinese Medicine In Recent 20 Years

Irritable bowel syndrome (IBS) is a common functional bowel disease, its etiology and pathogenesis on modern medicine have not been fully elucidated. Traditional Chinese medicine has made some progress in the diagnosis and treatment of IBS. But the differences of physicians’awareness on the etiology and pathogenesis, therapy, syndrome differentiation and treatment,effect Chinese medicine on the clinical experience of the disease summary. In this paper, drawing a variety of research ideas, nearly20journal articles published in domestic Chinese medicine treatment of IBS as the research object, using frequency analysis, correlation analysis, cluster analysis, and other data mining methods, analyzed IBS syndrome, symptoms, medication and other aspects,and made preliminary discussion for the syndrome differentiation and treatment regularity of IBS.Objective:Drawing on different ideas, data mining the syndrome differentiation and treatment law of IBS from the multi angle and level, in order to make useful exploration for perfecting the syndrome differentiation system of IBS, and provide a new way for the clinical diagnosis and treatment of IBS.Method:Comprehensively search and get1994-2013journal literature of TCM in the treatment of IBS. Through strict inclusion and exclusion criteria, the literature were selected to meet the requirements. Then leading into grade system of evidence of TCM clinical literature quality, the literature can be divided into6categories. Standardize the syndromes, symptoms, prescription drug name, etc, establish the "Chinese modern literature database" and input data. According to the target needs to choose different quality grades of literature, using a variety of methods of data mining to research, combined with traditional Chinese medicine theory, finally analyze the results and draw conclusions.Conclusion:Included in the journal of traditional Chinese medicine in the treatment of IBS with701references, syndrome differentiation and treatment (Ⅰ)96references, specially the treatment (Ⅱ)605references. Including Ⅰa9references, Ib0references, Ic69references, Id18references,Ⅱ a408 references, Ⅱb197references.1. Through the frequency analysis performed in quantitative measurement of the701TCM treatment of IBS literature, we can draw the following conclusions: In recent20years, the whole amount of the literature of TCM treatment of IBS has been a growing trend, and the amount will continue to grow in the future five years. The main research units concentrated in the southeast coastal economically developed areas and central research strength stronger provinces. The core author group has been formed but is still not stable, lack of high yield authors and research collaboration. The papers published by provincial journal of traditional Chinese medicine as the main front, research focused on the special treatment, the lack of randomized controlled trials of treatment based on syndrome differentiation, high level papers were fewer.2. By using the methods of frequency analysis, correlation analysis and clustering analysis, the study conducted the research to the nearly20years of syndrome differentiation and treatment IBS journal literature, extracted the syndrome elements11and syndrome target6, preliminary studied the evidence combination rule on the elements of syndrome, the targets of syndrome, and between them. On this basis, the paper furtherly explored the drug laws of the syndrome differentiation and treatment of IBS.3. By using the methods of frequency analysis, the paper studied drug laws of the nearly20years of the periodical literature of TCM in the treatment of IBS, we can draw the following conclusions:The occurrence of diarrhea predominant IBS with "Spleen Deficiency" as the core pathogenesis, the incidence of constipation IBS with "deficiency of large intestine fluid" as the core pathogenesis. Regardless of type of IBS gived consideration to the diarrhea-type and constipation-type, to a certain extent reflected the overall overview of TCM treatment, but dialectical prescription medication was not enough.4. Through the study of TCM syndrome of IBS, the common syndrome of irrespective type can be divided into7categories:liver stagnation and spleen deficiency syndrome, spleen kidney yang deficiency syndrome, spleen and stomach qi deficiency, liver qi stagnation, splenasthenic hygrosis syndrome, spleen stomach damp heat syndrome, jin yin deficiency syndrome. Diarrhea-predominant IBS grouped into7categories:liver stagnation and spleen deficiency syndrome, spleen kidney yang deficiency syndrome, splenasthenic hygrosis syndrome, spleen stomach qi deficiency, syndrome of intermingled heat and cold, damp heat in spleen and stomach, liver qi stagnation. IBS constipation is classified into five types:jin yin deficiency syndrome, spleen kidney yang deficiency syndrome, syndrome of stagnation of liver qi, deficiency of spleen qi and blood deficiency intestinal dryness.5. Through the study on the medication regularity of IBS, it reflected the use of drugs for treatment of IBS documents in diarrhea and constipation type, more flexible. Investigated the corresponding drug cases of syndromes and accompanied symptom in the regardless of type, diarrhea and constipation-type three dialectical therapy literature, and got the common syndromes corresponding drug compatibility, it was basic addition and subtraction, and seeing different symptoms, discretionary joined symptomatic treatment drugs generally.Innovation of this study:1. The study introduced quality evaluation methods of clinical medicine literature of TCM, built evidence hierarchy in line with traditional Chinese medicine journal literature, graded all of the literature included, and according to the target demand differences chose different quality grades of literature to research, to ensure the reliability of the research.2. The study reasonablely used "syndrome elements-targets-evidence combination rule " syndromes research ideas, combining the method of data mining, which was further extended to the study of drug laws. the laws of TCM drugs for IBS conducted in-depth discussion.3. IBS was divided into regardless of type, diarrhea and constipation type three categories. Through the use of a variety of data mining methods, the system research from syndromes and medication two aspects for the three respectively had been carried on, to investigate the dialectical medication rule of IBS from the multi-level and multi-angle.
