| Objective:In order to achieve a comprehensive inheritance of Professor Wang Qi’s academic thought,this study used chronic urticaria as an entry point and summarized the key points of his inheritance by combining quantitative and qualitative methods to provide reference for the academic inheritance of famous veteran Chinese medicine practitioners.Methods:1.A case series study was conducted to evaluate the clinical efficacy,safety and long-term efficacy of Professor Wang Qi’s empirical formula "Tuoming Xiaofeng Decoction"in the treatment of chronic urticaria.2.The clinical cases of chronic urticaria treated by Professor Wang Qi were systematically sorted out,and the mehod of data mining was adopted to explore the rules and ideas of clinical prescription medication.3.Using allergic diseases as an entry point,we used semi-structured interviews to explore Professor Wang Qi’s academic thinking on the treatment of allergic diseases and to draw out the points of TCM Heritage.Results:1.The study finally included 31 study subjects,9 men and 22 women,with an average age of 32.65±23.27 and a mean disease duration of 5 years,and 24 cases were taking antihistamines before treatment.The total effective rate after 4 weeks of treatment with Tuoming Xiaofeng Decoction was 80.65%,and the total symptom scores as well as individual scores decreased after treatment(P<0.01).The use of antihistamines was reduced after treatment,and some patients had stopped taking them,and no significant adverse reactions were complained during the treatment period.2.In this study,clinical cases of chronic urticaria treated by Professor Wang Qi between 2010 and 2022 were systematically reviewed,and a total of 132 cases were included.Through data mining,it was found that:①Basic information:chronic urticaria is predominantly female(61.89%),the age distribution is mainly between 31 and 40 years old.The regional distribution is mainly in northern China,and the onset is seasonal,mainly in spring,summer and autumn,with the majority of attacks occurring at night.②Medication pattern:the core drugs are Wu Mei,Chi Zhi,Zi Cao,Fang Feng,Bai Jili,Chan Yi,Gan Cao,Xu Changqing,Ye Jiaoteng,and Dong Guapi,etc.The efficacy is mainly based on heat-clearing,table-clearing and deficiency-supplementing drugs,and the four qi are mainly cold and mainly attributed to the liver meridian.A total of 7 groups of high-frequency drug pairs and 5 groups of core drugs were mined(see text for details).In addition,data mining revealed that Professor Wang Qi’s prescriptions used many famous prescriptions of meridians as well as herbs with special effects.3.A total of eight researchers were included in this study,and eight interview transcripts were obtained,totaling 25000 words,with reference to books and other literature.Through inductive analysis by coding at three levels of rooted theory,a total of five categories were finally obtained,which were the awareness of allergic diseases,prescription and medicine characteristics,treatment ideas,academic thinking and moral quality,and it was found that moral quality could override the other categories,mainly in 3 aspects:being a doctor,being a teacher and being a scholar.Conclusions:1.Professor Wang Qi’s experienced formula "Tuoming Xiaofeng Decoction" for the treatment of chronic urticaria can improve clinical symptoms and signs,reduce the use of antihistamines and has a high safety profile.2.Through data mining,we found that Professor Wang Qi’s treatment of chronic u rticaria was based on regulating allergic constitution,reflecting the diagnosis and treat ment model of "differentiation-constitution,differentiation-disease,differentiation-syndrom e" and the idea of "the main formula for the main disease",as well as the characteris tic of using the famous prescriptions of scriptures.3.The inheritance of TCM is a process of mutual achievement between teachers and students,and the essence of the inheritance lies in the inheritance of "moral qualities"(Dao),which has a spiritual leading role and contributes to the inheritance of medical skills. |
