Objective:To analyze the correlation between TCM syndromes of Non-specific infectious sinus tract and clinical data, and to explore the risk factors affecting multiple hospitalizations of patients with Non-specific infectious sinus tract, hoping to capture the relationship between TCM syndromes in clinical practice and clinical indicators of Western medicine, and provide guidance for clinical treatment accordingly,as well as advance intervention and targeted treatment for risk factors that may prolong the course of Non-specific infectious sinus tract disease. It also introduces the clinical experience of teacher Xuxuying in the treatment of sinuses, which provides some ideas for the treatment of sinuses with integrated Chinese and Western medicine. Methods:This study was a retrospective analysis of medical records.244 patients with Non-specific infectious sinus tract admitted to Beijing Traditional Chinese Medicine Hospital from January 2010 to December 2022 were selected, and 227 patients meeting the inclusion criteria were selected.Clinical data, data of four diagnoses of traditional Chinese medicine and culture results of pathogenic microorganisms of patients were collected.Firstly, according to the standard method of syndrome differentiation of Dou Dao guide,TCM syndrome types were unified, and the correlation between TCM syndrome types of Dou Dao and clinical data was analyzed.Then,181 patients with single hospitalization and 46 patients with multiple hospitalization were divided into groups, and the clinical data of the two groups were statistically analyzed. Finally, through clinical follow-up study, typical cases were recorded and Professor Xuxuying’s clinical experience in the treatment of sinus tract was summarized.Research results:1.227 patients with Non-specific infectious sinus tract were divided into 108 cases of lingering poison and 119 cases of deficiency of Qi and blood according to TCM syndrome types.Clinical data of the two groups were compared, including gender, age, past history(hypertension, diabetes, heart disease, surgical disease).Correlation analysis of admission data(WBC,RBC,Hgb,NEUT,CRP,ALB,ESR,strains)showed that the two groups were statistically significant only in terms of gender.2.Multivariate regression analysis showed that neutrophil count at first admission and Gram-negative bacterial infection were associated with multiple hospitalizations in Non-specific infectious sinus tract patients(P<0.05).3.Professor Xuxuying believed that the early, middle and late stages of the disease course of the sinus tract should be grasped. The general principle of syndrome differentiation is Yin and Yang, the combination of local and whole is emphasized, and the cause of the sinus tract is clarified.Treatment should grasp the early drainage patency, the middle stage inflammation subsided, the late tissue repair three links, according to different stages combined with the internal and external treatment of traditional Chinese medicine, eliminate the lesions, promote healing.Conclusions:1.According to the TCM syndrome type, the 227 patients with sNon-specific infectious sinus tract were divided into 108 cases of lingering toxin not clear and 119 cases of Qi and blood deficiency. The comparison between groups only had statistical significance in gender, while other clinical data had no statistical significance. It may be related to the disease attributes of the Non-specific infectious sinus tract itself. Therefore,in clinical diagnosis and treatment practice,it is necessary to grasp the pathogenesis evolution in the course of the disease and pay attention to the combination of the local and the whole when performing TCM syndrome differentiation in patients with the Non-specific infectious sinus tract.2.Risk factors for multiple hospitalizations in Non-specific infectious sinus tract patients:neutrophil count at first admission, and wound gram-negative bacterial infection.3.Professor Xuxuying treated patients by stages when differentiating sinus passages, flexibly applied internal and external treatment of traditional Chinese medicine, and achieved good clinical effect, which can provide ideas for clinical work of traditional Chinese surgery and researchers.
