Professor Zhang Mingxue's Study On The Syndrome Treatment And Network Pharmacology Of Coronary Heart Disease Combined With Hypertensio

Purpose:Based on the data mining method,this paper studies and summarizes the syndrome and treatment law of Professor Zhang Mingxue in the treatment of stable angina pectoris complicated with hypertension,and uses network pharmacology to explore the mechanism of core prescription in the treatment of stable angina pectoris complicated with hypertension,so as to provide basis for the treatment of this disease by traditional Chinese medicine.Materials and method:Collect the relevant data of patients with stable angina pectoris complicated with hypertension diagnosed and treated by Professor Zhang Mingxue in the Affiliated Hospital of Liaoning University of traditional Chinese medicine from January 1,2019 to June 30,2021,include 194 medical records according to the acceptance and discharge standard,extract the relevant data and establish a database;Using Excel,SPSS modeler 18.0and SPSS statistics 25,the prescription data were analyzed by descriptive statistics,association rules and cluster analysis.According to the above analysis results and combined with clinical practice,Professor Zhang’s academic thought and core prescription of syndrome differentiation and treatment of coronary heart disease complicated with hypertension were further summarized.The effective components and target information of the core prescription were screened through tcmsp and batman-tcm database,and the corresponding gene name of the target was obtained through Uni Prot database;The gene targets of stable angina pectoris and hypertension were obtained by genecards and disgenet disease gene database,and venny21 software,draw Wayne diagram and obtain the target of drug disease interaction;Through Cytoscape 3 9.0 software to draw the network diagram of "traditional Chinese Medicine-Composition-target";The protein-protein interaction network was constructed by string database;The target was enriched and analyzed by David and kobas database,and then the mechanism of core prescription in the treatment of stable angina pectoris complicated with hypertension was discussed.Results:1.Syndrome elements: the highest frequency of pathogenic syndrome elements in 194 patients was Qi deficiency(96.91%),followed by fire heat(94.33%),phlegm turbidity(94.33%),yin deficiency(33.51%),blood stasis(21.65%)and Qi Stagnation(8.25%).2.Medication situation: 194 prescriptions contain 121 traditional Chinese medicines,Among them,there are 13 traditional Chinese medicines with a frequency of more than 50%,which are Astragalus membranaceus,American ginseng,Pinellia ternata,Gualou,Xiebai,Poria cocos,bupleurum,Scutellaria baicalensis,Platycodon grandiflorum,cohosh,Gastrodia elata,uncaria,gardenia;Cold,warm and mild drugs are often used;The medicine is mainly sweet,bitter and pungent;Meridians are mostly lung meridians,liver meridians,spleen meridians and heart meridians;Drugs such as tonifying deficiency drugs,heat clearing drugs and phlegm resolving drugs are used more frequently.3.Cluster analysis: the drugs can be clustered into 8 categories.The first category is:Astragalus membranaceus,American ginseng,Pinellia ternata,Trichosanthes,Allium macrostemon;Class 2: Platycodon grandiflorum,Cimicifuga,bupleurum;Category 3: uncaria,Scutellaria baicalensis,Gastrodia elata and gardenia;Category 4: Prunella vulgaris,cassia seed,motherwort,Polygonum cuspidatum and Polygonatum;Category 5: Poria cocos and red peony;Category 6: keel,licorice;Category 7: Festuca arundinacea root;Category 8: wild jujube kernel.4.Association analysis: the stronger association rules are Magnolia officinalis and Fructus aurantii;Polygala tenuifolia,wild jujube kernel;Red peony,Poria cocos;Keel,licorice.5.Combined with the clinical experience of the tutor,the core prescriptions are American ginseng,Astragalus membranaceus,Pinellia ternata,Gualou,Xiebai,Gastrodia elata,uncaria,Scutellaria baicalensis and gardenia.6.There are 108 active ingredients in the core prescription of "treating different diseases together",including quercetin,kaempferol β-Sitosterol,stigmasterol and baicalein are the main effective components;There are 180 potential targets corresponding to drugs and diseases,mainly TNF,IL6,MMP9,Jun,AKT1,mapk3,CASP3,IL1 B,EGFR,TP53 and other targets;The biological processes of go enrichment analysis mainly involve hypoxia reaction,nitric oxide biosynthesis,protein phosphorylation,RNA polymerase II promoter transcription,inflammatory reaction,angiogenesis,aging,leukocyte migration,etc;KEGG pathway enrichment analysis mainly involved AGE-RAGE signaling pathway,fluid shear stress and atherosclerosis pathway in diabetic complications.Conclusion:The supervisor believes that the pathogenesis of stable angina pectoris combined with hypertension is always the asthenia in origin and asthenia in superficiality,and the treatment principle is to reinforce insufficiency and reduce excessiveness,and therapy with syndrome differentiation.It is mainly the method of Supplementing Qi,resolving phlegm,clearing liver and purging fire,the clinical prescription is Shenqi Gualou Xiebai decoction combined with Tianma Gouteng decoction in the treatment of stable angina pectoris complicated with hypertension.
