Comparative Study On Tongue Image Characteristic Parameters Of Colorectal Cancer With Damp-heat Accumulation Syndrome And Spleen-kidney Yang Deficiency Syndrom

Objective: In this study,TFDA-1 tongue diagnostic instrument was used to collect CRC dampness and heat deficiency syndrome,spleen and kidney,then extracted the tongue image,and then separated the tongue coating,and finally analyzed the analysis software(TDAS2.0)to obtain the characteristic parameters of tongue image.Comparative analysis of the tongue characteristic parameters of colorectal cancer dampness syndrome group,spleen and kidney Yang deficiency syndrome group and normal physical examination group shows the differences between the two syndrome types of colorectal cancer,which provides an objective reference index for the clinical diagnosis and syndrome differentiation treatment of colorectal cancer.Methods: From November 2020 to July 2021,108 patients with colorectal cancer(including 51 patients with dampness-heat accumulation syndrome and 57 patients with spleen and kidney Yang deficiency syndrome)were collected from Hunan Affiliated Hospital of Traditional Chinese Medicine,Hunan Research Institute of Traditional Chinese Medicine and Hunan Cancer Hospital,At the same time,102 cases of normal people were collected in the physical examination department of the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine;Basic information such as height,weight and age,The above subjects or their families are also requested to fill in the clinical health status assessment questionnaire scale(H20 scale)and the TCM clinical diagnosis record form(V2018),Establish a database of tongue image information parameters including basic information and four-diagnosis information;Upload the collected tongue images to the Information Intelligent Processing Laboratory of Shanghai University of Traditional Chinese Medicine,Tongue and coating were processed by TDAS2.0 analysis software,The corresponding tongue image characteristic parameters are obtained;last,Statistical comparative analysis of the tongue characteristics of the above three lingual groups using SPSS25.0,The differences in the tongue characteristic parameters between patients and normal people and between the damp heat accumulation syndrome and spleen and kidney Yang deficiency syndrome were obtained.Results:Compared with the normal group,the characteristic parameters of tongue image in the damp heat accumulation syndrome group were as follows: the parameter values of tongue color zhiclrr were significantly higher than those in the normal group(P < 0.01),the parameter values of zhig,zhiclrb,zhiclrg and zhiclrcb were significantly higher than those in the normal group(P < 0.05),and the parameter values of tongue color zhiclrs,zhiclrla,zhiclrlb and zhiclrcr were significantly lower than those in the normal group(P< 0.05);The parameters taiclrr,Taig and taiclrcb of tongue coating color were significantly higher than those of the normal group(P < 0.05),and the parameters taiclrs,taiclrla and taiclrcr of tongue coating color were significantly lower than those of the normal group(P < 0.05);The zhicon,zhient and zhimean parameters of tongue texture were significantly lower than those of the normal group(P < 0.05),and zhiasm parameters were significantly higher than those of the normal group(P < 0.05);The parameters taicon and taient of tongue coating texture were significantly lower than those of normal group(P < 0.05);The parameter perall of tongue coating thickness was significantly higher than that of the normal group(P< 0.01);Compared with the normal group,the characteristic parameters of tongue image in the spleen kidney yang deficiency syndrome group showed that the values of tongue color parameters zhig,zhiclrb,zhiclrg,zhiclrcb,zhiclri,zhiclrl and zhiclry were significantly higher than those in the normal group(P< 0.05),and the values of zhiclrs,zhiclrla,zhiclrlb and zhiclrcr were significantly lower than those in the normal group(P < 0.05);The tongue coating color parameters Taig,taiclrcb,taiclrb,taiclrri,taiclrl,taiclrcb were significantly higher than those in the normal group(P < 0.05),and the parameters taiclrs,taiclrla,taiclrlb,taiclrcr were significantly lower than those in the normal group(P < 0.05);The zhicon,zhient and zhimean parameters of tongue texture were significantly lower than those of the normal group(P < 0.05),and zhiasm parameters were significantly higher than those of the normal group(P< 0.05);The parameters taicon and taient of tongue coating texture decreased significantly,and the parameter perall of tongue coating thickness increased significantly compared with the normal group(P< 0.01);Comparison of tongue color parameters in two types of colorectal cancer: there were significant differences in the values of tongue color parameters zhiclrb,zhiclrr,zhiclrg,zhiclri,zhiclrl and zhiclry between the two groups(P < 0.01);Compared with the spleen kidney yang deficiency syndrome group of colorectal cancer,the zhiclrb and zhiclrr parameters in the damp heat accumulation syndrome group were significantly higher(P <0.05),and the zhiclrg,zhiclri,zhiclrl and zhiclry parameters were significantly lower(P <0.05);Comparison of tongue coating color parameters between the two types of colorectal cancer:there were significant differences in the values of tongue coating color parameters taiclrr,taiclrl,taiclrla and taiclrcr between the two groups(P < 0.05);Compared with the spleen kidney yang deficiency syndrome group of colorectal cancer,the taiclrr parameter value in the damp heat accumulation syndrome group was significantly higher,and the taiclrl,taiclrla,taiclrcr parameter values were significantly lower(P < 0.05).Comparison of tongue texture parameters between the two syndrome groups of colorectal cancer: there was significant difference between the two groups in tongue texture parameters zhicon,zhient and zhimean(P < 0.01).Compared with the spleen kidney yang deficiency group,the zhicon,zhient and zhimean parameters in the damp heat accumulation syndrome group were significantly higher(P < 0.01).Comparison of tongue coating texture parameters and tongue coating thickness parameters between two types of colorectal cancer: there was significant difference between the two groups in tongue coating texture parameters taicon and taient(P < 0.01).Compared with spleen kidney yang deficiency group,taicon and taien parameters in damp heat accumulation syndrome group increased significantly(P < 0.01);There was significant difference in the parameter perall of tongue coating thickness between the two groups(P <0.05).Compared with the spleen kidney yang deficiency group,the parameter perall of damp heat accumulation syndrome group was significantly higher(P < 0.05).Conclusion: The tongue color parameters zhi Clr B,zhi Clr R,zhi Clr G,zhi Clr I,zhi Clr L and zhi Clr Y were significantly different between the colorectal cancer dampness-heat accumulation syndrome group and the spleen and kidney Yang deficiency syndrome groups,Color parameters tai Clr R,tai Clr L,tai Clr La,and tai Clr Cr were significantly different between the two groups,The differences in the two groups of texture parameters,zhi CON,zhi ASM,zhi MEAN,zhi ENT,tai CON,and tai ENT,were statistically significant,The difference in per All between the two groups was statistically significant,It suggests that these characteristic parameters can be used as reference for syndrome differentiation of dampness-heat syndrome and Yang deficiency in spleen and kidney.
