Objective:To compare the expression levels of serum brain-derived neurotrophic factor(BDNF),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in patients with depression of liver-qi stagnation type and kidney-yang deficiency type,and to explore the difference of serum related factors between TCM syndromes of depression,so as to provide objective basis for the treatment of depression with traditional Chinese medicine.Research methods:1.In this study,the inclusion criteria and exclusion criteria of patients were determined with reference to the diagnostic criteria of depression in the fifth edition of the Diagnostic Statistical Manual of Mental Disorders.According to the team’s previous research on the syndrome of depression,we identified two common TCM syndrome types:liver-qi stagnation type and kidney-yang deficiency type,and established the TCM classification standard of depression in this study according to the recognized syndrome differentiation standard.two.The patients in this study were evaluated with Hamilton Depression scale(HAMD-24),Hamilton anxiety scale(HAMA),Young’s Mania rating scale(YMRS)and Social Disability screening scale(SDSS).And matched with healthy controls,venous blood samples were collected,centrifuged,the serum was packed separately and frozen at-80℃.The contents of brain-derived neurotrophic factor,tumor necrosis factor-α and interleukin-6 were detected by enzyme-linked immunosorbent assay(ELISA).Each serum marker was detected by the same batch of kit and operated by the same detection technician.3.SPSS22.0 software was used to analyze the differences of serum levels of brain-derived neurotrophic factor,tumor necrosis factor-α and interleukin-6 in depression group,healthy group,liver-qi stagnation depression group and kidney-yang deficiency depression group,and the correlation among serum brain-derived neurotrophic factor,tumor necrosis factor-α and interleukin-6.3.The data were statistically analyzed by SPSS22.0 software,including the differences of serum brain-derived neurotrophic factor,tumor necrosis factor-α and interleukin-6 in depression group,healthy group,liver-qi stagnation group and kidney-yang deficiency depression group,as well as the correlation between serum brain-derived neurotrophic factor,tumor necrosis factor-α,interleukin-6 and Hamilton 24-item scale score,Hamilton anxiety scale score and so on.Research findings:1.There was no significant difference in sex,age and education years between the control group and the depression group(P>0.05),and there was no significant difference in sex,age and education years between the kidney yang deficiency depression group and the liver qi stagnation depression group(P>0.05).2.Compared with the control group,the scores of SDSS and HAMA in the depression group were significantly higher than those in the control group,the scores of MoCA in the depression group were significantly lower than those in the control group,and the SDSS score in the depression group with deficiency of kidney yang was lower than that in the depression group with liver-qi stagnation,and there was significant difference between the two groups.3.Compared with the control group,BDNF was significantly lower,IL-6 and TNF-α were higher significantly in depression group,which proved that serum BDNF,TNF-α and IL-6 were related to the pathogenesis of depression,while the content of serum BDNF in depression group with deficiency of kidney yang was lower than that in depression group with liver qi stagnation,and there was significant difference between the two groups(P<0 05).The serum IL-6 in depression group with stagnation of liver qi was significantly higher than that in depression group with liver qi stagnation.Conclusion:The levels of serum BDNF,TNF-α and IL-6 in patients with depression are closely related to the pathogenesis and pathophysiology of depression.The expressions of serum BDNF,TNF-α and IL-6 in depression patients of different TCM syndromes are different.Serum BDNF is lower in kidney-yang deficiency type depression than liver-qi stagnation type,which is related to kidney main bone marrow and kidney refined blood.It can be used as an objective quantitative index of kidney-yang deficiency depression.Serum TNF-αand IL-6 have higher expression in liver qi stagnation,which is related to liver main drainage and smoothing of the whole body ventilator.It can be used as an objective quantitative index of liver-qi stagnation depression;serum BDNF level is negatively correlated with the HAMD scale score,serum TNF-α level is positively correlated with the HAMD scale score;serum BDNF level is negatively correlated with the HAMA scale score The serum TNF-α level was positively correlated with the HAMA scale score.
