Clinical Research On Lingnan Fire Acupuncture In Treating Diabetic Peripheral Neuropathy

Objective This topic takes diabetic peripheral neuropathy(DPN)patients as the research object,intervenes with Lingnan fire acupuncture and common acupuncture,compares the scores of TCM clinical symptoms and Toronto neuropathy score(TCSS score)before and after treatment,and observes the clinical efficacy and safety of the treatment of diabetic peripheral neuropathy with Lingnan fire acupuncture.In order to provide a simple and experimental treatment plan for the treatment of DPN patients.MethodsUsing a prospective randomized controlled study,66 inpatients or outpatients who met the inclusion criteria in the Department of Endocrinology and Acupuncture Department of the Guangzhou University of Traditional Chinese Medicine were divided into treatment group and control group according to the principle of random control(random number table method),33 cases in each group.The two groups of patients were given basic comprehensive treatment of western medicine(1.Basic diabetes treatment for the enrolled patients:(1)Diabetes health education,in cluding diabetes diet,exercise guidance;(2)According to the different blood sugar levels of each patient,choose the appropriate hypoglycemic plan: such as oral hypoglycemic drugs or subcutaneous injection of insulin;(3)blood sugar control range:fasting blood sugar <8.0mmol/L before breakfast,and blood sugar <11.1mmol/L2 hours after meal;2.If the patient is accompanied by other underlying diseases such as hypertension and hyperlipidemia,medication will be administered;3.Treatment of DPN: oral mecobalamin tablets,0.5mg/time,3 times a day,continuous treatment for 2 weeks).On the basis of the above comprehensive treatment,the treatment group received shallow Lingnan fire needling,and the control group received ordinary acupuncture.The acupoints of the two groups were the same: mainly the acupoints of the affected limb,Quchi,Shousanli,Hegu,and Waiguan for upper limb lesions;Yanglingquan,Zusanli,Sanyinjiao,Jiexi,and Taixi for lower limb lesions.Treatment course: 3 times a week,the course of treatment is 2 weeks,a total of 6 treatments,and the observation period is 4 weeks.Before and after treatment,TCM clinical syndrome scores,TCSS score,vibration threshold detection,and TCM clinical syndrome scores at the 4th week follow-up were recorded to evaluate the difference in curative effect between the two groups.Results1.Baseline data: 66 patients were included in this study,1 case was lost in the treatment group,2 cases were lost in the control group,and finally 63 effective cases were included,32 cases in the treatment group(Lingnan fire acupuncture group)and 32 cases in the control group(common acupuncture group).)of 31 cases,there was no significant difference in the baseline data(age,sex)between the two groups(P>0.05),which was comparable.2.Before treatment,the TCM clinical symptom score,Toronto clinical score(TCSS score),and vibration threshold were compared between the two groups.There was no statistical difference between the two groups(P>0.05),which was comparable.3.The total score of TCM clinical symptoms: compared with before treatment,there was a statistical difference in the distribution of TCM symptom scores between the two groups after the overall treatment(P<0.5),suggesting that both treatment schemes can improve patients’ TCM clinical symptom scores,and the effect of the treatment group is more obvious(P<0.05).4.TCSS score: The TCSS scores of the two groups before and after treatment were statistically different(P<0.05),suggesting that both treatment regimens could reduce the TCSS scores of the patients;compared between the groups,the TCSS scores of the treatment group(Lingnan Fire Needle group)were lower compared with the control group(common acupuncture group),there was a statistical difference(P<0.05).5.Threshold of vibration sensation: There was a statistical difference in the threshold of vibration sensation before and after treatment in the two groups(P<0.05),and there was no statistical difference between the two groups(P>0.05),suggesting that both treatments can improve the vibration sensitivity of patients.6.Overall curative effect: Taking TCM clinical symptom score as curative effect index,after treatment,the effective rate of the treatment group was 93.75%,and the effective rate of the control group was 77.42%.The overall curative effect of the two groups was statistically different(P<0.05).ConclusionBoth Lingnan fire needling and ordinary acupuncture can effectively improve the clinical symptoms of DPN patients,reduce TCSS scores,and improve vibration sensitivity,and Lingnan fire needling has a better curative effect in general.
