| ObjectiveTo investigate the status of dyadic coping,intimate relationships,anxiety and depression in breast cancer patients and their spouses,and analyze the correlation between dyadic coping,intimate relationship,anxiety and depression in breast cancer patients and their spouses.To explore the actor-partner effect of dyadic coping on anxiety and depression of breast cancer patients and their spouses from a dyadic perspective,and to examine the mediating role of intimate relationship.MethodsA convenience sampling method was used to select 270 breast cancer patient-spouse pairs from Shantou Central Hospital from May 2022 to December 2022 as the study population.General information questionnaires,Dyadic Coping Inventory,Lock-Wallace Marital Adjustment Scale,and Hospital Anxiety and Depression Scale were used to assess the general information,dyadic coping,intimate relationships,anxiety and depression levels of both patients and spouses.Data were entered,organized,and analyzed using SPSS 26.0 and Amos 26.0,including descriptive statistics,paired samples t-tests,Pearson correlation analysis,and actor-partner interdependence mediation analysis.Results1.The dyadic coping scores of breast cancer patients and their spouses were(116.41±10.07)and(119.22±11.34),respectively.There was a statistically significant difference between the two groups(t=-4.437,P<0.05).2.The total intimate relationship mean scores of breast cancer patients and their spouses were(102.43±14.06)and(99.19±15.12)>respectively.The percentage of patients and spouses with marital maladjustment(scores<100)was 36.67% and 43.33%,respectively.The marital adjustment level of patients was higher than that of their spouses(t=4.497,P<0.05).3.The anxiety and depression scores of breast cancer patients were(6.40±3.20)and(5.96±3.15),respectively.The detection rates of anxiety and depression(scores ≥8)were 29.64% and 27.41%,respectively.The anxiety and depression scores of spouses were(5.50±2.93)and(4.98±2.82),respectively.The detection rates of anxiety and depression were 23.70%and 20.00% respectively.The anxiety and depression levels of breast cancer patients were higher than those of their spouses(t=4.263,P<0.05;t=5.082,P<0.05).4.The anxiety and depression of breast cancer patients were negatively correlated with their own and their spouses’ dyadic coping,intimate relationships,and positive dyadic coping dimensions,and positively correlated with negative dyadic coping dimensions(all P<0.05).The anxiety and depression of spouses were negatively correlated with their own and patients’ dyadic coping,intimate relationships,and positive dyadic coping dimensions,and positively correlated with negative dyadic coping dimensions(all P<0.05).The dyadic coping,intimate relationships,anxiety,and depression of breast cancer patients were positively correlated with the corresponding variables of their spouses(all P<0.05).5.There were both actor(β=-0.192,P=0.002;β=-0.235,P<0.001 andβ=-0.145,P=0.023;β=-0.235,P<0.001)and partner(β=-0.274,P=0.009;β=-0.223,P<0.001 and β=-0.166,P<0.001;β=-0.157,P=0.013)effects of dyadic coping on anxiety and depression in breast cancer patients and their spouses.There were both actor(β=0.417,P<0.001 and β=0.433,P<0.001)and partner effects(β=0.342,P<0.001 和 β=0.341,P<0.001)between dyadic coping and intimate relationships of patients and their spouses.There were actor effects(β=-0.367,P<0.001;β=-0.372,P<0.001 and β=-0.475,P<0.001,β=-0.375,P<0.001)of intimate relationships on anxiety and depression in breast cancer patients and their spouses but no partner effects(β=0.056,P=0.360;β=-0.002,P=0.966 and β=0.009,P=0.880;β=-0.028,P=0.650).6.Both breast cancer patients and their spouses’ dyadic coping could affect their own anxiety(β=-0.153,95%CI=-0.071~-0.029,and β=-0.206,95%CI=-0.072~-0.038)and depression(β=-0.155,95%CI=-0.072~-0.028 and β=-0.162,95%CI=-0.057~-0.026)through their own intimate relationships.Both patients’ and spouses’ dyadic coping could affect their partner’s anxiety(β=-0.162,95%CI=-0.069~-0.030 and β=-0.125,95%CI=-0.052~-0.022)and depression(β=-0.129,95%CI=-0.053~-0.022 and β=-0.127,95%CI=-0.056~--0.019)through their own intimate relationships.Conclusions1.Nearly one-third of breast cancer patients and their spouses experience anxiety and depression,which deserves attention.The dyadic coping and intimate relationships of breast cancer patients and their spouses are at a medium-to-low level and need further improvement.2.The anxiety and depression of breast patients and their spouses are both influenced by their own and each other’s dyadic coping but only influenced by their own intimate relationships.The intimate relationships of patients and their spouses are both influenced by their own and each other’s dyadic coping.3.The dyadic coping of breast cancer patients and their spouses can have actor and partner effects on their own and each other’s anxiety and depression through the mediating role of intimate relationships. |
