To observe the characteristics of QT dispersion in patients with suspicious positive exercise test and to investigate the diagnostic value of QT dispersion in patients with suspicious positive plate exercise test.Methods:One hundred and thirty-three cases with suspicious positive results of the plate-exercise test performed at the People’s Hospital of Hebei Province from January 2017 to December 2021 were selected.General clinical data such as gender,age,hyperlipidemia,and blood and biochemical indices such as erythrocytes,hemoglobin,leukocytes,platelets,platelet mean volume distribution width,total cholesterol,triglycerides,high-density lipoprotein,low-density lipoprotein,apolipoprotein A,apolipoprotein B,lipoprotein a,and fasting glucose were collected.The QT values were measured before and after exercise and QTd and QTcd values were calculated.The patients were divided into coronary artery disease group and non-coronary artery disease group according to the coronary angiography results,and the differences between the above indexes of the two groups were compared.Results: 1.Comparison of general clinical data between the two groups,comparing the coronary artery disease group and the non-coronary artery disease group,showed that the prevalence of hyperlipidemia history,overweight and hypertension in the coronary artery disease group was significantly higher than that in the non-coronary artery disease group,with statistical differences(P < 0.05);the percentage of males in the coronary artery disease group was higher than that in the non-coronary artery disease group,with statistical differences(P<0.05).2.Comparison of blood and biochemical indexes between the two groups,showed that the coronary artery disease The comparison of blood and biochemical indexes between the two groups showed that the red blood cell count,hemoglobin concentration and fasting blood glucose values were significantly higher in the coronary artery disease group than in the non-coronary artery disease group,and the difference was statistically significant(P<0.05).3.The comparison of QTd values between the two groups before and after exercise in each time period showed that there was no significant difference between the QTd values of the coronary artery disease group and the non-coronary artery disease group before and50″after exercise(P>0.05),and 1′50″,2′50″,3′50″after exercise.The comparison of QTd values between the coronary and non-coronary groups at 1′50″,2′50″and 3′50″after exercise was statistically significant(P<0.05).4.The comparison of QTcd values between the two groups before and after exercise at each time period showed that there was no significant difference between the QTcd values of the coronary and non-coronary groups before and 50″after exercise(P>0.05),and the comparison of QTcd values between the coronary and non-coronary groups at 1′50″,2′50″and 3′50″after exercise was statistically significant(P<0.05).There was a statistically significant difference in QTcd comparison between the coronary group and the non-coronary group(P<0.05).5.The significance of QTcd values combined with risk factors for the diagnosis of coronary heart disease in patients with suspected positive exercise stress test was analyzed by multifactorial logistic regression,which showed that the subjects’history of hyperlipidemia(OR=2.570,95%CI=1.091-6.485,P=0.046),post-exercise 1′50″QTcd value(OR=1.009,95%CI=1.000-1.017,P=0.039),and 3′50″QTcd value(OR=1.010,95%CI=1.000-1.020,P=0.046)were three factors that influenced the positive correlates of coronary heart disease diagnosis.Conclusion:1.1′50″QTcd and 3′50′′QTcd after exercise are positive correlates of coronary heart disease diagnosis in patients with suspected positive exercise stress test,and have high value for the diagnosis of coronary heart disease.2.Hyperlipidemia is a positive correlate of coronary heart disease diagnosis in patients with positive exercise stress test,and the combined QT dispersion has high sensitivity to help the diagnosis of coronary heart disease in patients with suspected positive exercise stress test.The combined QT dispersion has a high sensitivity to help the diagnosis of coronary heart disease in patients with suspected positive exercise stress test.
