Background: Biliary tract cancer originated from the bile duct epithelium and is the second most common malignant tumor in the hepatobiliary system.Compared with other digestive tract malignancies(such as gastric cancer and colon cancer),the sensitivity of chemotherapy for biliary tract malignancies is extremely poor,which greatly limits its comprehensive treatment effect.In-depth study of the internal mechanism of low sensitivity of biliary malignant tumor cells to chemotherapy,and seek new methods to evaluate the chemosensitivity of biliary malignant tumors may provide new ideas for clinical improvement of the diagnosis and treatment of biliary malignant tumors.Method: In this study,the effect of chemotherapy on the level of mi R-433 and the effect of mi R-433 on the chemotherapeutic effect of biliary malignant tumor cells were determined by cell experiment;RT-PCR assay was used to detect the level of mi R-433 in serum samples from clinical patients to explore its feasibility as an indicator of sensitivity to chemotherapy for biliary malignancies.Result: RT-PCR experiment found that there was no significant difference in the expression of circulating mi R-433 in patients with biliary malignant tumors compared with digestive system tumors(mainly duodenal papillary carcinoma,colorectal carcinoma),cirrhosis,gallstones,choledocholithiasis,and gallstones,but the expression of circulating mi R-433 in serum samples of patients with middle and distal bile duct cancer,hilar bile duct cancer were higher than those of patients with pancreatic cancer,and had statistical significance;Although there is no significant difference in the expression level of serum mi R-433 between patients with good prognosis and patients with poor prognosis in biliary tract malignant tumors before chemotherapy,the level of serum circulating mi R-433 increased significantly after chemotherapy.Cell experiments showed that chemotherapeutic drugs induced the secretion of mi R-433 by biliary cancer cells,while hypoxia did not affect the expression level of mi R-433.Mi R-433 mimics promoted the chemoresistance of biliary cancer cells,and mi R-433 inhibitors could significantly reverse the chemoresistance of tumor cells.Conclusion: The serum circulating mi R-433 in patients with biliary malignant tumor did not increase significantly,but it has certain significance in differential diagnosis;The expression level of circulating mi R-433 is closely related to the prognosis of biliary malignant tumor after chemotherapy.
