Objective:Benign prostatic hyperplasia(BPH)is one of the most common diseases in men.With the increase of age,in patients with prostate volume will gradually increase,resulting in benign prostatic hyperplasia,if clinical fails to timely intervention will also lead to bladder detrusor excluded by a compensatory increase contraction force to the urine,but with the continuous extension of the course,obstruction can make the bladder detrusor decompensation,detrusor reshaping,injury bladder function,Subsequently,the bladder detrusor muscle has low contractility.Based on this,this study aims to investigate the incidence of benign prostatic hyperplasia(BPH)complicated with detrusor hypocontractile force(DU)at present,improve people’s understanding of detrusor hypocontractile force,analyze the risk factors of DU in BPH patients,and construct a nomogram prediction model for DU in BPH patients.Therefore,it can provide some reference for the early diagnosis and treatment of DU patients with high risk.Methods: 202 BPH patients who were treated in Guangzhou Hospital of Integrated Traditional Chinese and Western Medicine from June 2019 to June 2021 were selected as the model group,and 64 patients who were treated in Guangzhou Hospital of Integrated Traditional Chinese and Western Medicine from August 2021 to January 2020 were selected as the model group.patients as the validation group.Using the general clinical data of all patients,202 BPH patients were divided into DU group and non-DU group according to whether the BPH patients in the model group were complicated with DU.Univariate and multivariate Logistic regression was used to analyze the risk factors of BPH patients complicated with DU,and R(R3.5.3)was used to establish the risk nomogram model for predicting BPH patients complicated with DU.Results:(1)The average age of 202 BPH patients included in the model group of this study was(65.46±11.09)years old,of which 87 had DU,and the incidence rate of DU was 43.1%(87/202).Based on this,the model group was divided into two groups:DU group(87 cases)and non-DU group(115 cases);(2)BMI,residence,educational level,marital status,smoking history,drinking history,tea drinking,hypertension history,and urinary retention history of patients in DU and non-DU groups,detrusor hyperactivity,creatinine expression level,bladder capacity at initial bladder sensation,voided volume and residual urine volume,and other general data,there was no significant difference(P>0.05);while age,history of diabetes,nephrosis Compared with general data such as water history,IPSS score,PV volume,and PSA level,the differences were all statistically significant(P<0.05).High IPSS score,large PV and high PSA level were independent risk factors for DU in BPH patients(P<0.05).0.832(95%CI: 0.784~0.859);the calibration curve shows that the actual and predicted values of the model group and the validation group have a good fit,and the AUC values of the model group and the validation group are 0.790 and 0.811,respectively.Conclusion: Risk factors for DU in BPH patients include age > 60 years,history of diabetes,history of hydronephrosis,high IPSS score,large PV and high PSA level.During clinical treatment,medical staff can establish a rank line based on the above six risk factors.The graphical model can accurately predict the risk probability of concurrent DU in BPH patients.
