| Objective: The popularization of traditional Chinese medicine(TCM)health culture knowledge,the level of TCM health culture literacy,and its influencing factors were analyzed in Gansu Province,and the key areas and groups were identified to provide a scientific basis for the formulation of TCM related health policies and intervention strategies in Gansu Province.Methods: A stratified multi-stage combined random sampling method was used to extract the permanent population aged 15~69 years from 8 survey sites in Gansu Province,and the household survey was conducted using the ’ Chinese Citizens ’ Traditional Chinese Medicine Health Culture Literacy Questionnaire ’.A total of 2022 citizen was investigated in this study,and 1969 valid questionnaires were collected,with an effective rate of 97.38%.Among them,889(45.15%)were males and 1080(54.85%)were females,with an average age of(47.88±13.02)years.Epi Data 3.1 was used to establish a database,and the complex sampling module in SPSS 23.0 statistical software was used for statistical analysis.A descriptive analysis was made of the composition of basic information used by inquiry objects.The chi-square test was used to analyze the regional and population differences in the popularity of TCM health cultural knowledge and literacy level.The Logistic regression model and CHAID decision tree model were used to explore the influencing factors of residents ’ literacy level and compare the similarities and differences of the results.The random forest algorithm is used to rank the importance of each relevant factor.Results:(1)In 2019,the popularization rate,reading rate,trust rate,and acting rate of TCM health culture knowledge among residents in Gansu Province were 92.63%,88.53%,93.10%,and 53.18%,respectively.(2)When it comes to access to knowledge of TCM health culture,TV(90.4%),handheld terminals(68.4%),and the Internet(54.3%)ranked in the top three.Books(46.20%)and radio(41.24%)increased significantly.(3)The prevalence of TCM health literacy among residents showed the following characteristics: males were higher than females,urban residents were higher than rural residents and those without chronic diseases were higher than those with chronic illnesses.Family income is higher,and the popularity rate,reading rate,trust rate,and action rate are higher.The four rates for residents aged 65 to 69 were the lowest.Residents with pre-secondary education or higher have higher penetration,reading,trust,and action rates.In terms of occupation distribution,civil servants and teachers occupy the top two positions,while farmers are the lowest.(4)In 2019,the overall level of TCM health literacy of Gansu residents was 15.75%.Among the five dimensions of literacy,the basic concept of traditional Chinese medicine,the appropriate methods for the public,the healthy lifestyle,the common knowledge of health culture,and the ability to understand information were 26.08%,2.69%,30.06%,30.29%,and36.79%,respectively.(5)There were differences in the level of TCM health cultural literacy among residents of different urban and rural areas,gender,level of education,occupation,and whether suffering from chronic diseases(P<0.05).The five dimensions of literacy showed the following characteristics: the city is higher than rural,the male is higher than the female in the dimensions of basic concepts and TCM information understanding ability;young people are higher than old people.Except for the dimension of the public appropriate method,the highest peak value of each dimension was 25 ~ 34 years old.The higher the education level,the higher the quality level of each dimension;civil servants,medical staff,and teachers had higher literacy levels in all dimensions,while farmers had the lowest literacy levels in all five dimensions.Those with higher economic income had higher levels of the basic idea of medicine,healthy lifestyle,and cultural knowledge.The level of TCM information understanding ability of residents without chronic illness was higher than that of residents with chronic diseases.(6)Logistic regression analysis showed that occupation was the main influencing factor of TCM health cultural literacy.The analysis results of the CHAID decision tree model showed that occupation and urban and rural education levels were the factors influencing the level of residents’ health and cultural literacy of TCM.The results of the random forest showed that occupation had the greatest influence on residents’ health and cultural literacy level of TCM,followed by education level.Conclusion:(1)The elderly,heavy manual workers,people with low education levels,and low-income people are the key groups for the popularization of traditional Chinese medicine health culture knowledge and skill training in the future.(2)The level of traditional Chinese medicine health culture literacy of Gansu residents was low,and the difference between urban and rural literacy was significant.The literacy level of public appropriate methods was the lowest,and the literacy level of information understanding ability was the highest.(3)After taking into account the differences between urban and rural areas,the level of education,occupation,and gender are the influencing factors of TCM health culture literacy and the five-dimensional literacy of Gansu residents. |
