| Objective:1.Introduce the Attitude Scale for Nurses to Palliative Care of Older Adults,and conduct translation,back translation and cross-cultural debugging to form the Chinese version of the Attitude Scale for Nurses to Palliative Care of Older;2.Through item analysis and reliability and validity analysis,the psychological measurement indicators of the Chinese version of the scale were tested to improve the reliability and scientificity of the scale items;3.The Chinese version of the scale was used to investigate the current situation of geriatric nurses’attitude towards palliative care for the elderly,and to discuss and analyze the influencing factors of nurses’attitude towards palliative care for the elderly.Methods:Part One:The sinicization and validity test of the Attitude Scale for Nurses to Palliative Care of Older Adults.Firstly,obtain the authorized consent of the original author,According to the Brislin translation model,the original scale was translated,back-translated,cross-cultural debugging and pre-survey,and the Chinese version of the Attitude Scale for Nurses to Palliative Care of Older Adults was formed.A total of 600clinical nurses from A and B Grade A hospitals in Taiyuan,Shanxi Province from May to July 2022 were selected as the study objects,and the general data questionnaire and Chinese version scale were distributed to them to verify the reliability and validity of the Chinese version scale.Item analysis of Chinese version scale was carried out by correlation coefficient method and critical ratio method.Using internal consistency analysis,retest reliability evaluation scale reliability;Content validity and structure validity were used to measure the scale validity.Expert evaluation method was used to evaluate the scale content validity,and factor analysis was used to evaluate the structure validity.Part two:Current investigation.From August to November 2022,Using convenient sampling method,243 nurses working in geriatric department from 5 Grade A hospitals in Taiyuan,Shanxi Province were selected as the research objects.The general data questionnaire,the Chinese version of the Attitude Scale for Nurses to Palliative Care of Older Adults,and the palliative care quiz for nursing were used to investigate the current situation of geriatric nurses’attitude toward palliative care for the elderly and analyze its influencing factors.SPSS 26.0 software was used for data analysis,and general information of the research object was statistically described,expressed by frequency and percentage.Single factor analysis and multiple linear regression method were used to explore the influencing factors of geriatric nurses’attitude towards hospice care for the elderly.Results:1.A Chinese version of the Attitude Scale for Nurses to Palliative Care of Older Adults,which accords with Chinese cultural background and has good reliability and validity,has been preliminarily formed.The scale contains 25 items in 4 dimensions:willingness and responsibility for Palliative care for the elderly,willingness and responsibility for Palliative care centered on the elderly and their families,confidence in Palliative care,and confidence in cooperating with other staff in Palliative care.The correlation coefficients between the 25 items and the total score of the scale ranged from0.321 to 0.752(P<0.001).All the items in the Chinese version of the scale had statistically significant differences between the two groups(P<0.05).The total Crobach’sαcoefficient of the scale was 0.903,the Crobach’sαcoefficient of the four dimensions ranged from 0.768 to 0.875,and the retest reliability after 2 weeks was 0.90.The S-CVI is 0.966 and I-CVI ranges from 0.83 to 1.00.The results of exploratory factor analysis showed that four common factors were formed,and the load of all items on each common factor was>0.40.The cumulative variance contribution rate after rotation was74.155%.By empirical factor analysis,the 4-factor structure model has better fitting effect and fitting index:X~2/df=1.521,RMSEA=0.038,RMR=0.026,GFI=0.918,CFI=0.972,IFI=0.972,TLI=0.968。2.The total score of geriatric nurses’attitude towards palliative care for the elderly was(89.82±8.92),among which the score of"willingness and responsibility for palliative care for the elderly"was(34.19±3.57),and the score of"willingness and responsibility for palliative care centered on the elderly and their families"was(27.16±3.80).The score of"confidence in palliative care"was(17.43±2.39)and the score of"confidence in palliative care in cooperation with other staff"was(14.40±1.77).There was a positive correlation between the attitude of geriatric nurses and their knowledge of palliative care(r=0.773,P<0.001).Single factor analysis showed that,Age,length of employment,job title,experience in caring for terminally ill patients and their family members,loss of loved ones or friends in the past five years,palliative education or training,family members suffering from critical illnesses(e.g,cardiovascular disease,Cancer,dementia and other diseases)and whether or not they live with the elderly can influence geriatric nurses’attitudes toward palliative care for the elderly.Multiple linear regression analysis showed that 98.7%variation in the attitude of geriatric nurses to palliative care for the elderly could be explained by whether they had received hospice care education or training and the degree of palliative care knowledge(P<0.001).Conclusion:1.In this study,a Chinese version of the scale with good reliability and validity has been preliminarily formed by translating the Attitude Scale for Nurses to Palliative Care of Older Adults into Chinese and testing its reliability and validity.The Chinese version of the scale can be used to evaluate the attitude level of domestic geriatric nurses towards palliative care for the elderly and carry out analysis.2.Geriatric nurses have a positive attitude towards palliative care for the elderly,but the overall situation needs to be further improved.The results of this study showed that whether the nurses had received palliative care education or training,and the degree of palliative care knowledge were important factors influencing the geriatric nurses for palliative care of the elderly.In the future,nursing managers can improve the enthusiasm of geriatric nurses for palliative care by holding specialized nursing training. |
