Objective:By explore the relevant factors affecting the occurrence and prognosis of synchronous colorectal bi-primary carcinoma and mono-carcinoma,aim to provide a reference for the clinical diagnosis and treatment of patients with bi-primary carcinoma and mono-carcinoma.Methods:Our study collected the clinical data of 48 patients with bi-primary carcinoma,60 cases of mono-primary colon cancer and 60 cases of mono-primary rectal cancer who were treated in Shanxi Provincial Cancer Hospital from January 2015 to January 2018.The patients clinical information including gender,age,drinking history,smoking history,body mass index(BMI),carcinoembryonic antigen(CEA),CA199,hemoglobin,albumin,TNM stage.Due to the loss of follow-up of some patients,the final enrollment analysis included 45 cases of bi-primary carcinoma,53 cases of mono-primary colon cancer and59 cases of mono-primary rectal cancer.We used SPSS26.0 software to analyzing the clinical data of bi-primary carcinoma with mono-primary cancers.Kaplan-Meiera method was used to compare the bi-primary carcinoma with mono-primary cancer of overall survival(OS).Results:The average age of bi-primary carcinoma patients was 62.8±10.5 years,including 28 males and 17 females,with a male-to-female ratio of 1.65:1;the average age of mono-primary colon cancer patients was 60.9±12.1 years,including 30 males and 23 females,with a male-to-female ratio of 1.30:1;and the average age of mono-primary rectal cancer was 60.2±11.1 years,including 30 males and 29 females,with a male-to-female ratio of 1.03:1.There was a significant difference in BMI between patients with bi-primary carcinoma and mono-primary colon cancer(P = 0.041),but there were no significant differences in gender,age,alcohol history,smoking history,CEA,CA199,hemoglobin,albumin and TNM stage(P > 0.05).Compared with patients with mono-primary rectal cancer,patients with bi-primary carcinoma had significant differences in BMI(P = 0.023),CEA(P = 0.004)and CA199(P = 0.028),but there were no significant differences in gender,age,drinking history,smoking history,hemoglobin,albumin and TNM stage(P > 0.05).The 1,3 and 5 year overall survival rates of patients with bi-primary carcinoma were 95.56%,77.78% and 62.22%.The OS of mono-primary colon cancer were 94.34%,81.13% and 69.81%.The OS of mono-primary rectal cancer were 100%,88.14% and 72.88%,respectively.The OS was no significant differences between bi-primary carcinoma and mono-primary colon cancer(P = 0.764),also bi-primary carcinoma and mono-primary rectal cancer(P = 0.123).Conclusion:1.Compared with single primary carcinoma,patients with bi-primary carcinoma have a higher proportion of abnormal BMI,suggesting that abnormally elevated BMI may be associated with the risk of developing bi-primary carcinoma.2.In patients with bi-primary cancer,the tumor markers of CEA and CA199 were significantly higher than those of patients with mono-primary rectal cancer,but there was no significant difference compared with mono-primary colon cancer,which suggested that the elevation of CEA and CA199 were helpful to detect the possibility of rectal cancer may combined with colon cancer,so as to make a better individualized treatment strategy.3.The OS is not significantly different from bi-primary cancer and single primary carcinoma,suggesting that through early detection and make a appropriate treatment plan,bi-primary cancer patients can have a similar prognosis with mono-primary cancer.
