| Objective:To study the therapeutic efficacy and adverse effects of liposomal paclitaxel and docetaxel in neoadjuvant chemotherapy of breast cancer by the Meta-analysis and provide the reference for clinical treatment.Methods:A total of 6 Chinese and foreign databases were searched from CNKI database、VIP database、Wanfang database、Pub Med、Embase and Cochrane Library,and the search period was from the databases set up to February 2023,and literature collection and screening were carried out according to the established inclusion and exclusion criteria.Meta-analysis of the collected data was performed by Revman 5.4 software to compare the efficacy and safety of the two drugs for neoadjuvant chemotherapy of breast cancer.Results:A total of 12 studies,including 852 patients in the liposomal paclitaxel group and1248 patients in the docetaxel group,were included in the meta-analysis and showed that:the liposomal paclitaxel group had a higher p CR than the docetaxel group in the neoadjuvant chemotherapy of breast cancer [OR=1.58,95% CI(1.12～2.22),P=0.008<0.05].There was no significant difference in CR+PR between the two drugs for the neoadjuvant chemotherapy of breast cancer [OR=1.20,95%CI(0.93～1.56),P=0.16>0.05].In terms of safety,the incidence of leukopenia [OR=0.76,95%CI(0.61～0.95),P=0.02<0.05]、allergic reactions [OR=0.16,95% CI(0.06～0.41),P=0.0001<0.05]、skin and nail toxicity [OR=0.24,95%CI(0.18～0.58),P=0.0002<0.05]、alopecia [ OR=0.48,95%CI(0.31～0.74),P=0.001<0.05]and musculoarticular pain[OR=0.64,95%CI(0.48～0.84),P=0.002<0.05] in the liposomal paclitaxel group were lower than those in the docetaxel group.The incidence of thrombocytopenia [OR=0.70,95% CI(0.47～1.05),P=0.09>0.05]、peripheral neurotoxicity [OR=0.80,95% CI(0.60～1.06),P=0.12>0.05]、diarrhea [OR=0.84,95% CI(0.63～1.13),P=0.24>0.05]、nausea and vomiting [OR=0.84,95% CI(0.67～1.05),P=0.12>0.05]、the liver and kidney function injury [OR=0.79,95% CI(0.60～1.03),P=0.08>0.05] were not statistically significant between the two groups.Conclusion:In the efficacy of neoadjuvant chemotherapy of breast cancer,liposomal paclitaxel achieved a higher p CR than docetaxel;Liposomal paclitaxel have demonstrated a superior safety profile over docetaxel,especially in the incidence of leukopenia,skin nail toxicity,allergic reactions,musculoarticular pain,and Alopecia. |
