| ObjectiveBased on the social amplification theory of risk,this paper explores the risk warning capabilities that intravenous catheter maintenance online nurses should possess,and preliminarily constructs an indicator system for the risk warning capabilities of intravenous catheter maintenance online nurses.It verifies that this system serves as a scientific and reliable evaluation tool,providing a theoretical basis for the evaluation of intravenous catheter maintenance online nurses.Investigate the current situation of risk warning capabilities of intravenous catheter maintenance online nurses in Shandong region,analyze their influencing factors,and provide training ideas and objective basis for managers and clinical trainers.MethodsPart 1: Construction of a risk warning capability indicator system for intravenous catheter maintenance online nurses.Based on the social amplification theory of risk,literature research and semi-structured interviews were used to collect relevant information on the research topic,and a preliminary draft of the risk warning ability indicator system for intravenous catheter maintenance online nurses was drafted.Using the Delphi expert consultation method,the indicators were revised through two rounds of expert consultation to improve the risk warning ability indicator system of intravenous catheter maintenance online nurses.Using the Analytic Hierarchy Process(AHP),the weights and combination weights of various indicators were calculated using the yaahp10.3 software,and consistency testing was conducted to ultimately form a risk warning capability index system for intravenous catheter maintenance online nurses.Part 2: Based on the risk warning ability index system of venous catheter maintenance online nurses,a self-assessment questionnaire on the risk warning ability of venous catheter maintenance online nurses was developed.A questionnaire survey was conducted on venous catheter maintenance online nurses in Weifang area to test their reliability and validity.Part 3: Investigation on the current situation of risk warning ability of intravenous catheter maintenance online nurses.Select venous catheter maintenance online nurses from hospitals in Shandong region,and conduct a self-assessment questionnaire on the risk warning ability of venous catheter maintenance online nurses based on inclusion and exclusion criteria to understand the current situation and influencing factors of risk warning ability of venous catheter maintenance online nurses in Shandong region.Results1.This study conducted a literature review of 149 articles and conducted semi-structured interviews with 20 individuals to preliminarily construct a risk warning capability indicator system for intravenous catheter maintenance online nurses;Two rounds of expert consultation were conducted on 25 experts from 10 provinces/cities,with recovery rates of 83.33% and 96%,respectively.This indicates a high level of enthusiasm among experts.The authority coefficients(Cr)of the two rounds of expert consultation were 0.882 and 0.890,both greater than 0.7,indicating a high level of authority among experts.The Kendall coefficients(W)of the two rounds of expert correspondence were0.207 and 0.254 respectively(both P<0.01),indicating good coordination of expert opinions.The coefficient of variation of the first round of expert consultation ranged from0.000 to 0.1793,and the coefficient of variation of the second round of expert consultation indicators ranged from 0.000 to 0.1825.All indicators met the inclusion criteria,ultimately forming a risk warning capability index system for intravenous catheter maintenance online nurses,including three primary indicators: original risk warning capability,risk transmission warning capability,ripple effect warning capability,nine secondary indicators,and 39 tertiary indicators.Analytic Hierarchy Process results:The weight values of the three primary indicators of original risk warning ability,risk transmission warning ability,and ripple effect warning ability are 0.7172,0.1947,and0.0881,respectively;Among the secondary indicators,the top three in terms of combination weight are professional technical ability(0.4214),risk assessment ability(0.2319),and communication ability(0.1271).Among the three level indicators,the top six in terms of combined weight are the ability to practice intravenous therapy(0.2670),nursing intervention ability(0.0840),potential unexpected risk assessment ability of patients(0.0796),expression ability(0.0763),and theoretical knowledge ability of intravenous therapy(0.0705).The CR of each indicator is<0.1,indicating that the weight setting of the indicators is reasonable and consistent.2.Reliability and validity testing of the self-assessment questionnaire on risk warning ability of intravenous catheter maintenance online nurses: Cronbach’s α the coefficient is 0.971,the half reliability is 0.892,and the questionnaire item level content validity index(I-CVI)is 0.9487-1.0000.The exploratory factor analysis results showed that the KMO value was 0.902,and the Bartlett’s spherical test value was 11863.13(P<0.01).Principal component analysis and maximum variance orthogonal rotation methods were used to extract 8 factors with feature roots greater than 1,and the cumulative contribution rate of variance was 83.950%,which is consistent with the secondary indicator dimensions of the indicator system,indicating that the reliability and validity of the indicator system are good.3.The investigation on the status quo of the risk early warning ability of the contracted nurses in the vein catheter maintenance network in Shandong Province shows that the total score of the nursing risk early warning ability of the contracted nurses in the vein catheter maintenance network of "Internet plus nursing service" in Shandong Province ranges from 76-195 points,with an average score of 143.05 ± 17.49 points,which is at a medium level.The scores of each dimension from high to low are basic quality ability(17.00 ± 1.77),professional and technical ability(12.31 ± 2.32),communication ability(12.18± 1.97),propaganda ability(15.17 ± 2.19),preparation ability(11.16 ± 1.74),risk assessment ability(21.63 ± 3.66),safety management ability31.93 ± 4.64),critical thinking ability(12.70 ± 2.90),Legal practice ability(8.98 ± 2.26).The age of nurses,maintenance frequency,participation in risk training,and whether specialized nurses(PICC,intravenous therapy,tumor,blood)are the main factors affecting the risk warning ability of intravenous catheter maintenance online nurses.Conclusion1.Based on the social amplification theory of risk,this study uses the Delphi method to build an index system of risk early warning ability of intravenous catheter maintenance network appointment nurses,which is scientific and reliable,and provides a theoretical basis for network appointment nurses and nursing managers.2.The risk warning ability of vein catheter maintenance online nurses in Shandong region needs to be further improved.Nurses’ age,maintenance frequency,participation in risk training,and whether they are specialized nurses(PICC,intravenous therapy,tumor,blood)are the main factors affecting the risk warning ability of vein catheter maintenance online nurses. |
