| ObjectiveIn this paper,we analyze the correlation between debilitation,anxiety,coping style and quality of life of patients with multiple myeloma from the perspectives of debilitation,quality of life and its influencing factors based on a cross-sectional approach,study the correlation between debilitation,anxiety,coping style and quality of life of patients with multiple myeloma,use a self-regulation common sense model,construct structural equations,and explore the mediating effects of anxiety and coping style in the debilitation and quality of life of patients with multiple myeloma,with the aim of further alleviating The aim of this study is to further reduce the debilitation of patients with multiple myeloma,and thus to broaden the ideas and methods for improving the quality of life of patients.MethodUsing 195 patients with multiple myeloma hospitalized in a tertiary hospital in Qingdao from July 2021 to January 2022 as the study population,a cross-sectional survey study was conducted using the General Information Questionnaire,the Health Survey Short Form(SF-36),the Medical Coping Modality Questionnaire(MCMQ),the Tilburg Debilitation Index Scale,and the Self-Assessment Scale of Anxiety(SAS)to investigate patients’ life The quality of life,frailty,coping style and anxiety were investigated separately.The data were analyzed by SPSS 23.0 with frequency,percentage and description for counting and measurement data,respectively,and one-way ANOVA or independent sample t-test for comparison between groups;Pearson(normal distribution)or Sperman(non-normal distribution)was used for correlation analysis.Path analysis of the role between debilitation,quality of life,coping style and anxiety and depression in patients with multiple myeloma was analyzed using AMOS 24.0 software.Test level P <0.05 was considered a statistically significant difference.Result1.General data of 195 cases of multiple myeloma Results: The age range is 48 to 88 years old,with an average age of 63.81 ± 8.64 years;There were 97 males,accounting for49.70%,and 98 females,accounting for 50.30%;The number of people under 50 years old accounted for 5.10%,and the rest were over 50 years old;BMI ranged from 18.5 to23.9 in 80 cases,accounting for 41.0%;The number of married persons was 180,accounting for 92.3%;80% live together with their spouse,while the rest live alone or in groups with their children;Medical insurance type: 1.5% without medical insurance,and60% with rural medical insurance;There were 66 cases of concomitant diseases without other diseases,accounting for 57.4%,while the rest were accompanied by one or more diseases;87 cases(44.6%)had an onset time of less than one year.2.The debilitating status of 195 patients with multiple myeloma: The debilitating score was(8.44 ±2.98)points,at a moderate level,including a physical debilitating score of(4.69±2.09)points,a social debilitating score of(1.04±0.66)points,and a psychological debilitating score of(2.71±1.00)points;The results of univariate analysis showed that there was a statistically significant difference in the level of weakness among patients with different ages,living conditions,working conditions,and other diseases(P<0.05).The correlation analysis showed that weakness was positively correlated with anxiety(r=0.433,P<0.01),and negatively correlated with coping style(r=-0.630,P<0.05),and quality of life(r=-0.456,P<0.01).3.The score of quality of life was(51.44±10.11),and the score of each dimension was significantly lower than that of the normal model(P<0.01);Multiple linear regression showed that gender,medical expenses,living conditions,weakness,coping style-face and anxiety had predictive effects on quality of life(P<0.01).4.Structural equation results: the structural equation fit index was good and all indicators met the requirements(CMIN/DF=2.511,RSMEA=0.088,GFI=0.972,AGFI=0.917,IFI=0.969,CFI=0.968,NFI=0.950),Bootstrap method to verify the mediating effect,the low limit of the effect value to the high limit of the effect The results showed that debilitation had a direct effect on quality of life(β=-4.611 and an indirect effect(β=0.027),and the indirect effects were achieved through: debilitation→anxiety→coping-confrontation→quality of life,debilitation→coping-confrontation→quality of life,and debilitation→anxiety→quality of life.Conclusion1.debilitation in patients with multiple myeloma was at a relatively moderate level in this study,with age,residential status,work status and co-morbidities as the main influencing factors,and anxiety,nutritional status,and social support were correlated with the occurrence of debilitation.2.The quality of life of patients with multiple myeloma was at a low level,with gender,type of health insurance,and housing status as the main influencing factors,and anxiety,coping style-confrontation,and debilitation as predictors of quality of life.3.debilitation can affect quality of life directly or indirectly,and the latter is mainly influenced by anxiety and coping style-confrontation. |
