Objective:To investigate the effects of virtual reality technology on intraoperative anxiety,pain,and vital signs of patients undergoing transarterial chemoembolization under local anesthesia;to investigate the effects of virtual reality technology on the intraoperative physiology and psychology of patients undergoing transarterial chemoembolization under local anesthesia for the first time and patients undergoing transarterial chemoembolization under local anesthesia multiple times;to investigate the subjective care service satisfaction of patients undergoing transarterial chemoembolization under local anesthesia We also investigated the subjective care service satisfaction of patients undergoingt Transarterial chemoembolization under local anesthesia and their real feelings and experiences,and provided reference to understand the role of virtual reality technology for patients and optimize the application of virtual reality technology in surgery.Methods:Quantitative study: using convenience sampling method,74 patients with hepatocellular carcinoma who were hospitalized in the interventional department of a tertiary care hospital from April 2022 to October 2022 and required hepatic artery chemoembolization were selected as the study subjects according to the order of admission,and 37 patients with hepatocellular carcinoma who underwent transarterial chemoembolization in the interventional department of this hospital from April to June2022 were selected as the control group The 37 hepatocellular carcinoma patients who underwent transarterial chemoembolization in the interventional department of our hospital from July to October 2022 were selected as the test group.The control group used conventional care during surgery,and the test group used a virtual reality head-mounted display device on top of the control group until the end of surgery.The state anxiety scale was used to assess the patients’ anxiety status before the start of surgery and during surgery,and the digital graded pain scale was used to assess the patients’ intraoperative pain level,and the blood pressure,heart rate and satisfaction with surgical care before the start of surgery,during surgery,and at the end of surgery were recorded.Qualitative study: a descriptive phenomenological study method was used to conduct face-to-face semi-structured interviews with 12 patients wearing virtual reality technology during intraoperative transarterial chemoembolization using a purposive sampling method,and data analysis was performed using the Colaizzi seven-step method.Results:Quantitative study: there was no statistical difference between the two groups in terms of general demographic data and clinical data(P > 0.05).There were no statistical differences in blood pressure and heart rate before the start of surgery and at the end of surgery(P > 0.05);systolic blood pressure(130.84±15.63)and diastolic blood pressure(83.12±8.87)in the test group were significantly lower(P < 0.05)compared with systolic blood pressure(137.73±9.33)and diastolic blood pressure(87.41±5.10)in the control group at half an hour after the start of surgery,and There was no statistical difference in heart rate(P > 0.05);there was no statistical difference in blood pressure and heart rate in the test group compared with the control group at one hour after the start of surgery(P >0.05);there was no statistical difference in the anxiety status of patients before the start of surgery in the test group compared with the control group(P > 0.05),and the anxiety status of patients during surgery in the test group(43.57±5.53)compared with the control group(48.16 ± 6.47)was significantly lower(P<0.05);Intraoperative patient pain scores were significantly lower in the test group(3.62±1.75)compared to the control group(4.51±1.84)(p<0.05);Intraoperative anxiety status was higher in the control group of patients who had first local anesthesia and transarterial chemoembolization(52.89 ± 6.41)than in patients who had multiple local anesthesia and transarterial chemoembolization(46.64±5.81)(P<0.05);the surgical care satisfaction of patients in the test group was higher than that of patients in the control group.Qualitative research: three themes emerged:(1)positive effects of virtual reality technology interventions;(2)negative experiences of virtual reality technology interventions;and(3)the need for improvements in virtual reality technology.Conclusion:Virtual reality technology can effectively reduce intraoperative pain and anxiety levels in patients undergoing transarterial chemoembolization,and has the most significant effect on stabilizing preoperative blood pressure;the level of intraoperative anxiety is higher in patients undergoing local anesthesia and transarterial chemoembolization for the first time than in patients undergoing local anesthesia and transarterial chemoembolization multiple times;virtual reality technology is beneficial to improving the quality of care and enhancing patients’ satisfaction with surgical care;virtual reality technology can improve patients’ intraoperative anxiety,embarrassment,and other negative emotions,making the surgical procedure more relaxed and enjoyable;there are still some shortcomings in the process of use,and future use needs to be improved.
