Objective:Meta-analysis was used to study the risk factors for persistent ventricular arrhythmias in patients with hypertrophic cardiomyopathy,and to provide evidence-based medical evidence for the clinical management of patients at high risk of hypertrophic cardiomyopathy.Methods:Computer search Pub Med,Embase and Web of Science English database published literature on persistent ventricular arrhythmias in hypertrophic cardiomyopathy,the search period is from the establishment of the library to January 30,2023.Data from included articles were extracted,and the Newcastle Ottawa Scale(NOS)was used for literature quality evaluation,and the meta package in R 4.2.2 software was used for data analysis.Results:A preliminary search of 4620 articles and finally 22 articles were included,including a total of 13254 patients with hypertrophic cardiomyopathy,including 1280 cases of persistent ventricular arrhythmia.Meta-analysis showed late gadolinium-enhanced(LGE)[HR=1.34,95% CI: 1.03-1.75,P=0.03],left ventricular end-diastolic diameter(LVEDD)[HR=1.06,95% CI: 1.03-1.09,P<0.01],family history of SCD [HR=1.14,95% CI: 1.06-1.23,P<0.01],non-sustained ventricular tachycardia(NSVT)[HR= 1.48,95% CI,1.16,1.89,P<0.01],history of unexplained syncope[HR=2.20,95% CI,1.54-3.14,P<0.01] were risk factors for persistent ventricular arrhythmias in patients with hypertrophic cardiomyopathy(P<0.05).Conclusion:Advanced gadolinium-enhanced(LGE),left ventricular end-diastolic diameter(LVEDD),family history of SCD,non-sustained ventricular tachycardia(NSVT),and history of unexplained syncope are risk factors for persistent ventricular arrhythmias in patients with hypertrophic cardiomyopathy,which may provide valuable advice for the management of implantable cardioverter defibrillator(ICD)implantation in people at high risk of hypertrophic cardiomyopathy.
