Objective:By comparing the expression levels of peroxisome proliferator-activated receptor γ(PPARγ)in tissues of patients with eosinophilic chronic rhinosinusitis with nasal polyps,patients with non-eosinophilic chronic rhinosinusitis with nasal polyps and patients in the control group,the role of PPARγ in the occurrence and development of different intrinsic types of CRSw NP was preliminarily discussed,in order to provide a new direction for the clinical treatment of CRSw NP and explore more effective treatment strategies.Methods:In this study,we divided nasal polyp tissue specimens from patients with CRSw NP treated by nasal endoscopy who attended the Department of Otolaryngology-Head and Neck Surgery at the First Hospital of Shanxi Medical University from September 2022 to November 2022 into 20 groups.These patients were also divided into 2 groups based on the eosinophil/neutrophil(b ENR)ratio in peripheral blood:one group was the ECRSw NP group with 6 cases,and the other group was the non-ECRSw NP group with 14 cases.The control group consisted of 10 mucosal tissue specimens from the inferior turbinates of patients diagnosed with nasal septal deviation or maxillary sinus cysts who underwent nasal endoscopy during this period.Morphological changes of mucosal tissues in each group were observed by hematoxylin-eosin staining(HE staining)technique;IHC technique and q RT-PCR technique were also applied to detect the expression of PPARγin each tissue to compare the differences between different tissues.Results:1.The results of HE staining showed that the epithelium and basement membrane of the nasal mucosa of CRSw NP were significantly thickened and cupped cells were proliferated compared with the control group,while a large infiltration of inflammatory cells was seen in the ECRSw NP group compared with the non-ECRSw NP group,while the degree of eosinophil infiltration was more significant in the ECRSw NP group.(Figure 1 a-f)2.IHC results showed that we could observe that PPARγ positive cells showed brownish-yellow staining and were expressed in mucosal epithelial cells,glandular epithelial cells and inflammatory cells.PPARγ was expressed in nasal polyps and normal nasal mucosa,and the expression levels of ECRSw NP group and non-ECRSw NP group were significantly decreased(both P<0.05).At the same time,the expression of ECRSw NP group was lower than that of non-ECRSw NP group(P<0.05).(Figure 2-4)3.The m RNA expression of PPARγ was detected in all cases by q RT-PCR.Among them,the m RNA expression concentration of PPARγ was significantly reduced in the ECRSw NP and non-ECRSw NP groups,and the statistical values between the CRSw NP and normal control groups were all significant(P<0.05).In addition,in the CRSw NP group,the m RNA expression level of PPARγ in the ECRSw NP group was lower than that in the non-ECRSw NP group(P<0.05).(Figure 5)Conclusion:The expression of PPARγ in CRSw NP was significantly lower than that in normal control group.The expression of PPARγ in ECRSw NP was significantly lower than that in non-ECRSw NP.PPARγ may be involved in the occurrence and development of ECRSw NP,which provides a new idea for the further study of the pathogenesis of different types of CRSw NP,suggesting that activation of PPARγ may provide a new path for clinical treatment of ECRSw NP.
