Association Of Interleukin-6 Polymorphisms With Coronary Artery Disease Susceptibility And Coronary Vasculopathy Severity In Diabetes Mellitus

Objective:To investigate the relationship between serum interleukin-6(IL-6)gene polymorphism and coronary atherosclerotic heart disease(CAD)in patients with diabetes mellitus(DM)susceptibility and the severity of coronary artery disease in diabetic patients with CAD.Methods:A total of 104 patients with diabetes mellitus admitted to the first hospital of Shanxi Medical University between January 2019 and June 2021 were collected,and the patients were divided into diabetes mellitus alone(27 patients)and diabetes mellitus with CAD(77 patients)based on clinical information and the results of coronary angiography,which in turn were divided into low-volume(40 patients),medium volume(15 patients),and high-volume(22 patients)based on the values of coronary syntax scores.Serum IL-6,lipids,glycosylated hemoglobin(Hb A1c),and IL-6 rs1800795(-174 G/ C),rs1800796(-572 G / C),and rs1800797(-597 G /A)genotypes of the patients were tested and analyzed for association.Serum IL-6gene polymorphisms were tested in the 2 groups to analyze the susceptibility of different genotypes to coronary artery disease in diabetic patients and the correlation between the degree of coronary artery stenosis in diabetic patients with CAD.Results:1.Analysis of IL-6 gene polymorphism and susceptibility to CAD in diabetes patients: Compared with GG genotype,CC genotype of IL-6 rs1800795(P=0.036)was associated with an increased risk of CAD in diabetes patients,with an OR(95%CI)of 9.20(1.16-73.3).At the same time,compared with the G allele,the C allele(P=0.002)of IL-6 rs1800795 was also associated with an increased risk of CAD in diabetes patients,with an OR(95% CI)of 4.71(1.77-12.56).Compared with the CC genotype,the GG(P=0.021)and GC(P=0.025)genotypes of IL-6 rs1800796 were associated with an increased risk of CAD in diabetes patients,with an OR(95% CI)of 4.96(1.27-19.32)and 3.24(1.16-9.04),respectively.Compared with the C allele,the IL-6 rs1800796 allele G(P=0.03)was associated with an increased risk of CAD in diabetes patients,with an OR(95% CI)of 2.92(1.45-5.87).2.Association analysis of IL-6 gene polymorphisms with different coronary stenosis degree groups in diabetes mellitus with CAD: CC genotype(P=0.028)of IL-6 rs1800795 was associated with increased risk of coronary stenosis degree in diabetes mellitus with CAD patients compared with GG genotype with OR(95% CI)was 4.63(1.18-18.21).Compared with allele G,allele C(P=0.024)of IL-6rs1800795 was associated with the degree of coronary artery stenosis in diabetes mellitus patients with CAD,with an OR(95% CI)was 2.50(1.13-5.55).Compared with the CC genotype,the GG(P=0.004)genotype of IL-6 rs1800796 was associated with an increased risk of the degree of coronary stenosis in diabetic patients with CAD,with an OR(95% CI)was 9.35(2.06-42.31).Among the rs1800796 gene locus,compared with allele C,the G allele(P=0.013)was associated with the degree of coronary stenosis in diabetes mellitus patients with CAD,with an an OR(95% CI)was 2.46(1.21-5.00).The genotype distribution at the IL-6 rs1800797 locus was not statistically significantly associated with the degree of coronary stenosis in diabetic patients with CAD.But compared with allele A,allele G(P<0.001)was associated with the degree of coronary stenosis in diabetes mellitus patients with CAD,with an OR(95% CI)was 39.49(12.29-126.98).Conclusion:1.The CC genotype and allele C of IL-6 rs1800795 locus were correlated with the susceptibility to CAD and the degree of coronary artery stenosis in diabetes patients.2.The GG,GC genotype and allele G of IL-6 rs1800796 locus were associated with the susceptibility to CAD in diabetes patients.Moreover,GG genotype and allele G are associated with the increase of coronary artery stenosis in diabetes patients with CAD.3.The GG and GA genotypes of IL-6 rs1800797 locus have no correlation with the CAD susceptibility of diabetes patients and the degree of coronary stenosis of diabetes patients with CAD.At the same time,the allele G has statistical significance with the degree of coronary stenosis of diabetes patients with CAD,but we believe that the sample results lack the wild AA genotype,so the results have no clinical significance.
