Objective Investigate the predictive value of the Alberta Stroke Program Early CT Score(ASPECTS),based on the im STROKE artificial intelligence software,in the prediction of I.V.thrombolysis in acute ischemic stroke(AIS).Methods In this study,baseline data were collected from patients who met the enrolment criteria and were followed up for three months after admission to Baotou Central Hospital for acute cerebral infarction from 1 November 2021 to 30 October 2022.Modified Rankin Scale(mRS)scores were assessed and categorised into good prognostic group(mRS≤ 2)and poor prognostic group(mRS > 2).Using the NCCT ASPECTS score as the reference standard,the intraclass correlation coefficient(ICC)was used to evaluate the consistency of im STROKE ASPECTS and NCCT ASPECTS,RAPID ASPECTS and NCCT ASPECTS,and im STROKE ASPECTS and RAPID ASPECTS.Variables with P < 0.05 were included in the multivariate logistic regression analysis to screen for risk factors that could independently predict a poor outcome of intravenous thrombolysis in patients with AIS in order to compare baseline data between the good and poor prognosis groups.ROC was used to assess predictive value of im STROKE ASPECTS score on outcome of IV thrombolysis in patients with AIS.Results(1)With the expert consensus NCCT ASPECTS score as a reference standard,Good agreement between the im STROKE ASPECTS score and the NCCT ASPECTS score,ICC =0.794(95%CI:0.720~0.850,P<0.001);Good agreement between the RAPID ASPECTS score and the NCCT ASPECTS score,ICC = 0.822(95%CI:0.758~0.871,P<0.001);Good agreement between the im STROKE ASPECTS score and the RAPID ASPECTS score,ICC =0.828(95%CI:0.765~0.876,P<0.001).(2)Age at admission,NHISS score,time from presentation,platelet distribution,history of diabetes mellitus,and im STROKE ASPECTS score differed between the favorable and unfavorable groups(P<0.05).(3)In patients with acute ischaemic stroke,age,NHISS score at admission,time to thrombolysis,and im STROKE ASPECTS score had a significant impact on outcome(P<0.05).(4)Using ROC curve analysis,the AUC of im STROKE ASPECT to predict the outcome of IV thrombolysis in acute ischaemic stroke was 0.887 with a cut-off of 7.5 points.Conclusions 1)Artificial intelligence software im STROKE has a certain degree of clinical application value in the auxiliary evaluation of ASPECTS score.2)Age,admission NIHSS score,time to thrombolysis,RDW and im STROKE ASPECTS score were independent predictors of a 3-month poor outcome in intravenous thrombolysis patients with acute ischemic stroke.3)For the assessment of the clinical prognosis of patients with acute ischaemic stroke,the im STROKE ASPECTS score has a good predictive value.
