| Objective:1.To investigate the clinical efficacy and relative safety of the shallow fire-needle acupuncture stimulation plus cupping compared with oral famciclovir + gabapentin in the treatment of acute herpes zoster and the modulating effect of shallow fire-needle acupuncture stimulation plus cupping on serum 5-HT;2.To explore whether there is a combined effect between the shallow fire-needle acupuncture stimulation plus cupping and famciclovir.Methods:The 121 patients with acute herpes zoster of stagnated heat in liver meridian were randomly divided into 3 groups.Shallow fire-needle acupuncture stimulation plus cupping group: this treatment was given once every other day for 4 times;Drug control group: oral famciclovir + gabapentin.Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group: shallow fire-needle acupuncture stimulation plus cupping treatment every other day;famciclovir: 0.25g/time,3 times/day.The course of treatment is 7 days.To compare changes in VAS score,symptom-sign score,revised composite efficacy,5-HT level,analgesic requirements,adverse effects,and incidence of postherpetic neuralgia among groups and differences between groups,and the correlation between symptom-sign score and serum 5-HT level in the Shallow fire-needle acupuncture stimulation plus cupping group was analyzed.Results:1.VAS score decreased in all groups,the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group was superior to both the Shallow fire-needle acupuncture stimulation plus cupping group and the Drug control group(P<0.01),and the Shallow fire-needle acupuncture stimulation plus cupping group was superior to the Drug control group(P<0.01).2.All the symptom-sign score decreased compared with before,the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group was better than the Shallow fire-needle acupuncture stimulation plus cupping group and the Drug control group(P<0.01),and the Shallow fire-needle acupuncture stimulation plus cupping group was better than the Drug control group(P<0.01).3.Revised combined efficacy: The efficacy of the three groups was,in order,better in the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group(99.94%),the Shallow fire-needle acupuncture stimulation plus cupping group(94.9%),and the Drug control group(90.23%),with statistically significant differences(P<0.01).4.5-HT level decreased in all three groups,the decreased values in the Shallow fire-needle acupuncture group and the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group were better than Drug control group(P<0.01);no statistically significant difference between the Shallow fire-needle acupuncture group and the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group(P>0.05).5.The incidence of posterior neuralgia was significantly higher in the Drug control group(36.6%)than in the Shallow fire-needle acupuncture group(15.4%)and the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group(12.2%),P<0.05.6.The analgesic requirement was higher in the Drug control group(63.4%)than in the Shallow fire-needle acupuncture stimulation plus cupping combined with famciclovir group(36.6%)and the Shallow fire-needle acupuncture group(35.9%),P<0.05.7.The incidence of adverse reactions was lower in the Shallow fire-needle acupuncture stimulation plus cupping group than in the Drug control group(P<0.01).8.The symptom-sign score of patients in the Shallow fire-needle acupuncture stimulation plus cupping group was correlated with 5-HT levels(r= 0.759,P=0.000).Conclusion:1.Shallow fire-needle acupuncture stimulation plus cupping is more advantageous compared to drugs in the clinical treatment of acute herpes zoster,while reducing the use of analgesic drugs and the occurrence of adverse reactions and posterior neuralgia,and its effect may be related to its ability to reduce the level of 5-HT;2.There is a combined effect between shallow fire-needle acupuncture stimulation plus cupping and antiviral drugs. |
