| Objective:This study explored the intervention effect of "strengthening earth to generate metal" theory guiding acupoint massage in patients with acute exacerbation of COPD(syndrome of turbid phlegm obstructing lung),comprehensively evaluated patients’ clinical symptoms,lung function indexes and quality of life,and provided clinical basis for the effectiveness of "strengthening earth to generate metal" theory guiding acupoint massage in acute stage of chronic obstructive pulmonary disease(syndrome of turbid phlegm obstructing lung).Method:In this study,76 inpatients in the acute exacerbation of COPD(syndrome of turbid phlegm obstructing lung)who met the nanation standard from November 2021 to September 2022 in the pulmonary department of a third-grade A Hospital of Traditional Chinese Medicine in Kunming City,Yunnan Province were randomly divided into the observation group and the control group with 38 cases in each group according to the computer random generation number method.Patients in both groups were given routine treatment and nursing(including drug therapy,health exercise,diet care,emotional care,health education),the control group was given routine acupoint massage according to the doctor’s advice,and the points selected were double Dingchuan points,double Feishu points and double Zhongfu points as the main points,Tiantui point,double Fengmen points,double Chize points and double Kongzui points as the matching points.The observation group was given acupoint massage guided by the theory of "cultivating soil and producing gold" according to the doctor’s advice.The acupoints selected were double Dingchuan points,double Feishu points and double Zhongfu points,and double Pishu points,double Zu Sanli points,double Fenglong points and double Yin Lingquan points as matching points.Two groups of patients acupoint massage time was once a day,20min/time,1 week as a course of treatment,continuous intervention for 2 weeks,a total of two courses of intervention.TCM syndrome scores,TCM syndrome efficacy,COPD assessment test(CAT score)and lung function indicators were selected as evaluation tools,and CAT scores of the two groups of patients before intervention,1 week and 2weeks of intervention were statistically analyzed.The TCM syndrome scores and lung function indexes of the two groups were statistically analyzed before intervention and after 2 weeks of intervention,and the TCM syndrome efficacy of the two groups of patients after 2 weeks of intervention was statistically analyzed.Result:1.Information of included subjects:A total of 76 subjects were included in this study,and 4 cases fell off during the2-week intervention,with a shedding rate of 5.26%.A total of 72 cases were finally completed in this study,of which 36 cases were completed in the observation group and2 cases were shed,and 36 cases were completed in the control group and 2 cases were shed.2 Comparison of baseline data:Before intervention,there was no significant difference in the general data and clinical efficacy index between the two groups(P>0.05),indicating balanced comparability.3 Comparison of TCM syndrome scores:Before and after intervention,the scores of cough,sputum,wheezing,chest tightness,phlegm,abdominal distension,fatigue and fatigue and the scores of TCM syndrome of the two groups were statistically significant(P<0.001).After 2 weeks of intervention,the scores of cough,phlegm,wheezing,chest tightness,phlegm retention,abdominal distension,fatigue and fatigue and the scores of TCM syndroms in the two groups were compared between the two groups.The scores of cough,phlegm retention,abdominal distension,fatigue and fatigue and the scores of TCM syndroms in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The difference was not statistically significant(P>0.05).4 Comparison of curative effect of TCM Syndromes:After 2 weeks of intervention,there was a statistically significant difference in the efficacy of TCM syndrome between the two groups(P<0.05).The total effective rate of TCM syndrome efficacy in the observation group was 91.7%,and that in the control group was 83.3%.The proportion of clinical control and obvious effect in the observation group was higher than that in the control group.5 Comparison of CAT scores:Comparison of CAT scores between the two groups showed statistical significance in both time effect and interaction effect(P<0.05),but no statistical significance in intergroup effect(P>0.05).Intragroup comparison showed that CAT scores of 1 and 2weeks after intervention were lower than before intervention,with statistical significance(P<0.001).The CAT score of the observation group was slightly lower than that of the control group after 1 week of intervention,and the difference was not statistically significant(P>0.05).CAT score of the observation group was lower than that of the control group at the second week of intervention,and the difference was statistically significant(P<0.05).6 Comparison of lung function level:After 2 weeks of intervention,FEV1,FEV1%Pred and FEV1/FVC lung function indexes were increased compared with before intervention,the differences were statistically significant(P<0.001).There were no significant differences in FEV1,FEV1%Pred and FEV1/FVC lung function indexes between 2 groups(P>0.05).Conclusion:1.Acupoint massage guided by the theory of "strengthening earth to generate metal" can reduce the TCM syndrome score of patients with COPD acute stage(phlegm-turbidity obstructive pulmonary syndrome),effectively improve clinical symptoms and improve the efficacy of TCM syndrome.2.The theory of "strengthening earth to generate metal" guides acupoint massage to select acupoints through the idea of "treating lung and spleen together".It can not only effectively promote the relief of cough and sputum symptoms,but also regulate spleen and stomach transportation,improve the symptoms of dilatancy and abdominal distension,and reduce the feeling of fatigue and fatigue,which has more advantages than conventional acupoint massage.3.Acupoint massage guided by the theory of "strengthening earth to generate metal" can reduce CAT scores of patients with COPD acute stage(syndrome of turbid phlegm obstructing lung)and effectively improve the health status of patients,and its efficacy is superior to conventional acupoint massage.4.The effect of acupoint massage guided by the theory of "strengthening earth to generate metal" in improving wheezing,chest tightness and pulmonary function indexes of patients with AECOPD(syndrome of turbid phlegm obstructing lung)is comparable to that of conventional acupoint massage.The advantages of acupoint guided by the theory of "strengthening earth to generate metal" compared with conventional acupoint massage cannot be evaluated in a short time. |
