| Objective:To compare the safety and efficacy of propofol and remimazolam in painless abortion,and to explore the optimal dose of remazolam for anesthesia in painless abortion patients.Methods:A total of 120 patients undergoing painless abortion under intravenous anesthesia were randomly divided into four groups:propofol group（group P）,remimazolam low-dose group（R1 group）,remimazolam medium-dose group（R2group）,and remimazolam high-dose group（R3 group）.After intravenous injection of12μg/kg affintanil for analgesia,the four groups were intravenously administered1.5mg/kg of propofol in P group,0.1mg/kg of remimazolam in R1 group,0.15mg/kg of remimazolam in R2 group,and 0.2mg/kg of remimazolam in R3 group,and surgery was performed after the eyelash reflex disappeared.If the patient’s intraoperative movement reaction affects the operation of surgery,the salvage anesthesia regimen is to add propofol 0.5mg/kg/time in group P and 2.5mg/time in group R.The mean arterial pressure（MAP）,heart rate（HR）,blood oxygen saturation（Sp O2）,respiratory rate（RR）,BIS value（bispect ral index）and modified alertness/sedation（MOAA/S）scores were recorded before anesthesia（T0）,eyelash reflex loss（T1）,cervical dilation（T2）,and awakening（T3）,and the occurrence of adverse reactions were recorded.Results:Compared with the general data of the four groups,age,height,weight,gestational age and ASA grade were not statistically significant（P>0.05）;the MAP,HR,Sp O2,RR and RR of the four groups were lower than those in T0（P<0.05）,and the fluctuation range of P group was significantly higher than that of R1,R2 and R3groups（P<0.05）,and the MAP and HR of R1 group and R2 group were significantly higher than those in R3 group when dilating cervix（P<0.05）;The BIS values of group P were statistically significant compared with R1 group,R2 group and R3group（P<0.05）,and the difference between R3 group and R1 group and R2 group was statistically significant（P<0.05）,the MOAAA/S score of R3 group was statistically significant compared with R1 group and R2 group（P<0.05）,;P and the awakening time of R3 group was statistically significant compared with that of R1group,R2 group and R3 group（P<0.05）,compared with R1,R2 and R3 groups,there was no significant difference（P>0.05）;P the incidence of hypotension,hypoxemia,dizziness and drowsiness in the R1 group,R2 group and R3 group was higher（P<0.05）,the incidence of body movement in R1 group was higher than that in P,R2 group and R3 group,and the difference was statistically significant（P<0.05）,and the difference between R2 group and R3 group was not statistically significant（P>0.05）.Conclusion:In painless abortion,0.2 mg/kg remimazolam combined with 12μg/kg affentanil provides safe and effective sedation with fewer adverse effects,making it a better anesthesia regimen... |
