| Objectives The clinical application effects of two different kinds of peripheral venous catheters in patients with difficult venous access were compared and analyzed,including the safety,economy and comfort of the two groups of catheters in clinical application,so as to find out which peripheral venous catheters are safer and more effective and more suitable for clinical application.Methods This study was a cohort study,and the relevant data of patients receiving Mini midline catheter and PIV in surgery were collected from 2021.12-2022.12 in a third-class hospital in Wuhan in strict accordance with the standard of insertion.214 patients were eventually included,among which 107 cases were in Mini-midline catheter group and PIV group,respectively.The success rate of puncture,catheter indwelling time,catheter complications,catheter comfort and catheter-related economic cost of each patient were compared between the two groups from admission to discharge.Results1 A total of 107 patients were collected in this study from Mini-midline group and PIV group,respectively.The average age of patients in Mini-midline group was about(64.7±16.9)years old,and the average age of patients in PIV group was about(60.9±16.5)years old.There were 50 males in Mini-midline group and 52 males in PIV group.There was no significant difference in education level,BMI,surgery,occupation,medical payment method,smoking,drinking,daily infusion amount,red blood cell,leukocytes,albumin,D-dimer,complications,infusion of mannitol,iron sucrose and blood products between the two groups(P > 0.05).2 The results showed that the success rate of single catheterization in Mini-midline group(71%)was significantly higher than that in PIV group(43.9%),and the difference between the two groups was statistically significant(P <0.001).Compared with PIV group,the complications during and after catheterization in Mini-midline group were lower,with statistical difference between the two groups(P<0.001).The incidence of difficulty in catheter delivery in Mini-midline group was higher than PIV group,but the incidence of hematoma in PIV group was higher than Mini-midline group.As for the complications after catheterization,the main complications in Mini-midline group and PIV group were extravasation,phlebitis and tube blockage,However,the incidence of three major complications was lower in Mini-midline group(4.7%,0.9%and0.9%,respectively)than PIV group(48.6%,24.9%and 7.3%,respectively),and the difference between two groups was statistically significant(P<0.001)3 The results showed that the total score of comfort in Mini-midline group was significantly higher than PIV group,and the median score of comfort after catheterization in the two groups was 80 and 63 points,respectively,and the difference between the two groups was statistically significant(P<0.001).4 The comparison between the Mini-midline group and the PIV group showed that Mini-midline group was higher than PIV group in catheter placement cost,catheter maintenance cost and total catheter cost,and the difference between the two groups was statistically significant(P<0.001).In terms of complication treatment cost,Mini-midline group was lower than PIV group,and there was a statistical difference between the two groups(P<0.001).ConclusionsAfter catheter,the main complication of Mini-midline group and PIV group was extravasation,phlebitis and tube blockage,but the complication rate of Mini-midline group was lower than that of PIV group.The mean catheter indentation time in Mini-midline group was longer than PIV group.The comfort condition of the two groups was statistically different,and the Mini-midline group was better than PIV group.Compared with PIV group,the cost of catheter placement,catheter maintenance and total catheter cost of Mini-midline group was higher than that of PIV group,and the cost of complications treatment was lower than that of PIV group.Compared with PIV,Mini-midline group has the advantages of fewer complications,longer indenture time and higher comfort after catheter.For patients with difficult venous access,Mini-midline is recommended in clinic. |
