| Research background and purpose:Background: Primary liver cancer is one of the most common malignant tumors in China,and also the second most lethal tumor.At present,the number of patients with liver cancer is increasing.In clinical practice,the effect of general conditions,especially individual factors such as nutritional status,immune capacity,inflammatory status,is often ignored by focusing on the characteristics of the tumor to evaluate the prognosis.The controlled nutritional status score(CONUT)is used to calculate the nutritional status of patients by testing the blood samples of tumor patients.Because of its simple and fast advantages,it has been widely concerned by people.Studies have shown that the CONUT score has important predictive value in digestive tract malignant tumors.It can not only evaluate the nutritional status,but also predict the recovery after treatment.So does the CONUT score play a role in evaluating and predicting the prognosis of hepatocellular carcinoma after TACE treatment? Objective: To evaluate the relationship between preoperative CONUT score and prognosis of hepatocellular carcinoma after TACE treatment.Materials and Methods:The clinical data of 103 patients with transcatheter arterial chemoembolization(TACE)admitted to Yibin First People’s Hospital from December 2012 to December2017 were retrospectively analyzed,and the CONUT score was calculated,and the patients were grouped according to the cut-off value of the CONUT score(3 points).Classify the collected data and use the classification variables χ 2 Test for evaluation,calculate survival curve and univariate analysis by Kaplan-Meier method.In order to further determine the impact of multiple confounding factors on prognosis,use Cox proportional risk model to input relevant important variables in univariate analysis into multivariate analysis.Result:1.The group with CONUT score ≤ 3 had better overall survival time than the group with CONUT score>3(p<0.05);2.In the single-factor analysis results of OS in one year,the CONUT score is related to OS,while the multi-factor analysis is not an independent influencing factor;3.In the multi-factor analysis results of 3-year OS and 5-year OS,CONUT > 3points is an independent risk factor;4.In the multifactorial analysis of 5 years of OS,drinking history,AFP ≥400ng/m L,and CNLC stage were independent risk factors;5.CONUT > 3 is an independent risk factor for OS 3 and 5 years after TACE in CNLC IIIa and IIIb stages.Conclusion:The preoperative CONUT score can effectively predict the long-term prognosis of HCC patients with stage III of CNLC after TACE;... |
