| Research background and purpose:The study is aimed to understand the malnutrition risk status of inpatients with schizophrenia,and to analyze the influencing factors of malnutrition risk in inpatients with schizophrenia,and to explore the influence mechanism of sleep quality on malnutrition risk of inpatients with schizophrenia.Materials and Methods:This study was conducted in Chengdu,Sichuan Province from March to July,2022.310 inpatients with schizophrenia were selected by convenient sampling.The contents of the survey include:general demographic data,clinical disease data,short-form mini nutritional assessment,self-rating depression scale,pittsburgh sleep quality index and frailty phenotype.Excel and SPSS26.0 software were used for data entry and statistical analysis.Descriptive analysis of the basic data of the respondents;Harman single factor test is used to test whether there is common method deviation in the research data;Independent sample t-test and?~2-test were used to analyze the differences of variables between normal nutrition group and malnutrition risk group.Pearson correlation analysis was used to explore the correlation between sleep quality score,depression score,frailty score and malnutrition risk score.Binary logistic regression was used to explore the influencing factors of malnutrition risk.Process V3.3 was used to verify the regulated mediating effect model.Result:1.Among 310 inpatients with schizophrenia,114 patients were at risk of malnutrition,accounting for 36.77%.2.The results of univariate analysis showed that there were statistically significant differences in the BMI,living style,activity,fall history,sleep quality score,depression score and frailty score between the normal nutrition group and the malnutrition risk group(P<0.05).3.Correlation analysis showed that there was at positive correlation among sleep quality score,depression score and frailty score(P<0.01),and the scores of sleep quality,depression,frailty and malnutrition risk score were negatively correlated(P<0.01).4.Multivariate analysis showed that fall history(OR=1.896,95%CI:1.034~3.477)was a risk factor for malnutrition risk.Poor sleep quality(OR=1.114,95%CI:1.023~1.214)is a risk factor for malnutrition risk.Depression(OR=1.038,95%CI:1.003~1.075)is a risk factor for malnutrition risk.5.The intermediary test results showed that depression plays a partial mediating role between sleep quality and malnutrition risk,and the mediating effect accounts for15.17%of the total effect.The regulatory action results showed that the first half of the mediation model was adjusted by frailty,and sleep quality score had statistical significance in predicting depression score at high and moderate levels of frailty,but it had no statistical significance in predicting depression at score at low level of frailty.The critical value of frailty is-0.8306(data standardization).When frailty was exceeded this value,the correlation between sleep quality and depression is statistically significant and gradually increases with the increase of frailty.Conclusion:Malnutrition risk and sleep problems are widespread in inpatients with schizophrenia,and the influencing factors of malnutrition risk in these patients include fall history,sleep quality and depression.Depression mediates the relationship between sleep quality and malnutrition risk.In the indirect way that sleep quality affects malnutrition risk through depression,sleep quality has a stronger predictive effect on depression of schizophrenic patients in weak state.It is suggested that inpatients with schizophrenia complicated with sleep problems can be treated with measures to improve depression and frailty at the same time to promote the improvement of their nutritional status. |
