Objective:To investigate the relationship between serum creatinine to cystatin C ratio(CCR)in patients with type 2 diabetic kidney disease.Methods:A total of 640 patients with T2DM admitted to our department between April 2021 and April 2022 were enrolled in this study.According to whether had diabetic kidney disease,they were divided into diabetic disease group and diabetic kidney disease group.Compared and collected their clinical data and serological test results.The CCR value was calculated according to the peripheral blood test results,and the correlation between CCR and type 2 diabetic kidney disease was statistically analyzed using SPSS25.0 software.Results:Ⅰ.The gender,age,course of diabetes,hypertension,cerebral infarction,hyperuricemia,history of lower extremity arteriosclerosis,Cr,CysC,CCR,eGFR,Glu0min,HbAlc,and ESR of the two groups were statistically significant(p<0.05).2.Spearman correlation analysis results of male and female groups showed that CCR was negatively correlated with age,DM duration,HbA1C,CRP,ESR and ACR in T2DM population(p<0.05).There was a positive correlation between C peptide 0min and BMI in male subjects(p<0.05).There was no significant correlation with TC,TG,HDL-C and LDL-C.In all subjects,CCR was negatively correlated with Glu0min(p<0.05),but there was no significant correlation after male and female groups were divided.3.Two-class logistic regression analysis showed that low CCR(CCR<76.23),age,history of hypertension,history of hyperuricemia,hyperglycosylated hemoglobin,and increased erythrocyte sedimentation rate were the risk factors for diabetic kidney disease in T2DM patients.4.After the CCR values were divided into three equal quantiles,the multi-class logistic regression analysis showed that the OR value of microalbuminuria in the CCR middle quantile array was 5.372(95%CI 3.236~8.920),the OR value of microalbuminuria in the CCR middle quantile array was 12.934(95%CI 2.897~57.784),and the OR value of microalbuminuria in the CCR low quantile array was 13.000(95%CI 7.720~21.891),The OR value of large amount of albuminuria in CCR low quantile array was 44.474(95%CI 10.303~191.981).Conclusion:CCR is closely related to diabetic kidney disease in patients with T2DM.The low value of CCR is a risk factor for the occurrence of albuminuria in T2DM patients.CCR decreases with the increase of ACR in T2DM patients.CCR is expected to become a personalized risk assessment indicator for diabetes patients with nephropathy.
