| objective By analyzing the correlation between serum aldosterone level and low voltage zone of left atrium in patients with persistent atrial fibrillation,to explore whether preoperative serum aldosterone level has predictive value of low voltage zone of left atrium.Methods A total of 60 patients who were admitted to Fuyang People’s Hospital from March 2021 to March 2022 to undergo radiofrequency ablation for persistent atrial fibrillation were selected.The preoperative serum aldosterone level and intraoperative low voltage zone of the left atrium were collected,and the patients were divided into low-voltage zone group and non-low-voltage zone group according to whether there was intraoperative low voltage.Multivariate binary Logistic regression analysis was performed on the factors with statistically significant differences to analyze the independent risk factors for the left atrial low-voltage zone,and ROC curve was drawn to evaluate whether aldosterone level had predictive value for the left atrial low-voltage zone.The relationship between the level of aldosterone and the area of left atrial low voltage area was analyzed by linear regression.Results A total of 60 patients with persistent atrial fibrillation were enrolled,including 34 patients(56.7%)in the low-voltage region group and 26 patients(43.3%)in the non-low-voltage region group.There were no significant differences in age,sex,body mass index,hypertension,type 2 diabetes,coronary heart disease,stroke/transient ischemic attack,and CHA2DS2-VASC score between the low-voltage and non-low-voltage groups(P > 0.05).Serum aldosterone level,disease course > 1 year,left anteriorposterior diameter >45mm in low-voltage group was higher than that in non-low-voltage group,the difference was statistically significant(P < 0.05).Multivariate logistic regression analysis showed that high serum aldosterone level,disease course greater than 1year,left atrial anteriorposterior diameter greater than 45mm(P < 0.05)were independent risk factors for left atrial low voltage region.ROC curve analysis results showed that preoperative serum aldosterone level predicted the area under the curve of left atrial with low voltage region was 0.811(P < 0.05),and the optimal cut-off value of preoperative serum aldosterone calculated by Yoden index was 157ng/dl,which predicted the sensitivity of 76.5% and specificity of96.3% for left atrial with low voltage region.Linear regression analysis showed that the higher the aldosterone level was,the larger the area of left atrial low voltage region was(normalization coefficient was 0.777,P < 0.01).conclusions In patients with persistent atrial fibrillation,the level of serum aldosterone is an independent risk factor for whether there is a low voltage area in the left atrium,and it can be used as an index to predict whether the left atrium has a low voltage zone in patients with persistent atrial fibrillation,and the higher the serum aldosterone level is,the larger the low voltage area is. |
