Study On The Law Of Treatment Of Danggui Shaoyao Powder Based On Ancient And Modern Medical Cases

Objective:Danggui Shaoyao Powder comes from the ’ Golden Chamber ’written by Zhang Zhongjing in the Eastern Han Dynasty.It is a representative prescription for the treatment of abdominal pain during pregnancy and abdominal pain in women with liver and spleen disorders,blood deficiency and blood stasis,and water retention and dampness obstruction.The application of Danggui Shaoyao Powder in modern clinical practice is not limited to women’s abdominal pain,but also involves internal,external,gynecological,facial,skin and other diseases.By collecting and sorting out the ancient medical records of Danggui Shaoyao Powder and the modern medical records of the past 20 years in China,this topic uses data mining technology to analyze the disease name,symptoms,drugs and other information of the ancient and modern medical records of Danggui Shaoyao Powder and summarize the characteristics of the ancient and modern prescriptions of Danggui Shaoyao Powder,so as to provide a more complete application index and theoretical basis for the clinical application of Danggui Shaoyao Powder.Methods:Through the retrieval of ’ Chinese Medical Code ’ software,access to Heilongjiang University of Chinese Medicine Library,Hainan Medical College Library and Hainan Provincial Library related books to obtain ancient medical records;the modern clinical medical records from January 2003 to December 2023 were obtained by searching the literature databases of traditional Chinese medicine related journals such as China National Knowledge Infrastructure,Wan Fang,and VIP.According to the established standards,the data tables of ancient and modern medical records were established respectively.Frequency analysis,golden section rate,association rules and cluster analysis were used to analyze the data of disease name,symptoms and drugs in medical records,so as to summarize the relationship between prescriptions and syndromes and the application rules of Danggui Shaoyao Powder in ancient and modern medical records.Results:A total of 44 ancient medical records were included in this study,and 29 cases were recorded by gender,including 6 males and 23 females,with a ratio of 1:4.A total of 30 diseases were treated.Dysentery,tympanites,multiple abscess,and irregular menstrual cycle,mainly involving gynecological diseases,spleen and stomach diseases,and hepatobiliary diseases.A total of 75 symptoms,a total of 167 times,the highest frequency of symptoms of abdominal pain,followed by irregular menstruation,fever,fatigue,chest tightness.In ancient medical records,the frequency of use of angelica,peony,tuckahoe and largehead atractylodes rhizome was higher,and the frequency of use of chuanxiong and alisma was less than that of the four flavors.There were 89 kinds of flavored drugs in ancient medical records,involving 16 kinds of classification.The most frequent use of tonic drugs was 15 flavors,with a total of 77 times.Followed by qi medicine,a total of 14 flavors,a total of 45 times.The main flavored drugs are tangerine peel,ginseng,astragalus,prepared rehmannia root,cinnamon.There were 45 males and 228 females in 273 modern medical cases,and the ratio of male to female patients was about 1:5.There were 175 young patients,accounting for 64.10%.240 cases recorded the onset season,including 79 cases in spring and 64 cases in summer.There are 66 kinds of TCM disease names in modern medical records.The high-frequency TCM disease names are women’s abdominal pain,edema,dysmenorrhea,excessive movement of fetus,abdominal pain,diarrhea and vertigo,mainly involving gynecological diseases and kidney diseases.There were 105 kinds of western medicine names involved.The high-frequency western medicine names were pelvic inflammatory disease,dysmenorrhea,abnormal uterine bleeding,idiopathic edema and threatened abortion,mainly involving obstetrics and gynecology diseases and urinary system diseases.There were 117 kinds of symptoms,1658 times.The most frequent symptom was abdominal pain,145 times in total,accounting for 53.11%.The other high-frequency symptoms were fatigue,loss of appetite,loose stool and edema.In the medical records of abdominal pain,93 cases explained the pain site,and the lower abdomen was 86 times,accounting for 92.47%.96 cases explained the nature of pain,including 59 times of dull pain and 27 times of distending pain,accounting for 87.51%.In the tongue color record,the frequency of pale tongue,pale red tongue and red tongue was 76.42%,which was the core tongue color.In the tongue shape record,the frequency of tooth-marked tongue and fat tongue is 72.52%,which is the core tongue shape.In the records of moss,the frequency of white moss and thin moss is 62.96%,which is the core moss.In the pulse record,the frequency of pulse string,pulse thin and pulse sinking was 67.88%,which was the core pulse.In modern medical records,the frequency of use of the six drugs in the original prescription is equivalent.The most commonly used drugs are angelica 10 g,peony 15 g,tuckahoe 15 g,chuanxiong 10 g,largehead atractylodes rhizome 15 g,alisma 10 g.A total of 201 drugs were added,involving 19 categories.The number and frequency of tonic drugs were the highest,with a total of 32 flavors and 231 times of use,followed by blood-activating and stasis-removing drugs,qi-regulating drugs and heat-clearing drugs.The main flavored drugs are astragalus,motherwort,bupleurum,cyperus rotundus,coix seed.In the correlation results between symptoms,the highest degree of support was less menstrual flow-abdominal pain,and the highest degree of improvement was more vaginal discharge-white vaginal discharge.Among the correlation results between symptoms and tongue and pulse,the highest support was abdominal pain-white fur;the results of the association between symptoms and modified drugs showed that the highest degree of support was Shenpi fatigue-Huangqi,and the highest degree of improvement was edema-abdominal skin.In the results of the correlation between tongue and pulse and modified drugs,the highest degree of support was motherwort-fine pulse,and the highest degree of improvement was scutellaria baicalensis-greasy fur.The results of cluster analysis of highfrequency drugs were divided into two categories.The first category was the original prescription drugs,and the second category was the flavored drugs,which had the effects of regulating qi and relieving pain,activating blood circulation and dredging channels,promoting diuresis and eliminating dampness.The results of cluster analysis of high-frequency symptoms were divided into three categories.The first category was abdominal pain,the second category was fatigue and loss of appetite,and the third category was loose stool,edema,dizziness and other symptoms of liver depression and spleen deficiency,blood deficiency and dampness.Conclusion:1.According to the analysis of ancient and modern medical cases,it can be seen that Dengue Shatoya Powder is mainly used in gynecological diseases including abdominal pain,dysmenorrhea,fetal movement,etc.,in ancient times,it was also used in hepatobiliary and spleen and stomach diseases such as diarrhea and abdominal pain.2.The pathogenesis of Dengue Shatoya Powder is liver depression and spleen deficiency,blood deficiency and blood stasis,water retention.The main symptoms of ancient and modern medical cases are abdominal pain,which is characterized by dull pain and distending pain.It is often accompanied by symptoms such as fatigue,loss of appetite,loose stools,edema,and chest tightness.3.The composition of Danggui Shaoyao Powder is stable,and the six drugs are closely related,and the whole prescription drugs play the role of liver and spleen regulation and blood and water treatment through compatibility.Clinically,it is often combined with astragalus,motherwort,bupleurum,cyperus rotundus,coix seed and other tonic drugs,qi-regulating drugs and blood-activating and stasis-removing drugs to soothe liver and strengthen spleen,activate blood and nourish blood,and promote water and dampness.
