Objective:To compare the vacuum sealing drainage combined with oxygen infusion irrigation to treat infectious wounds after orthopedic surgery and conventional VSD combined with normal saline irrigation on infected wounds after orthopedic surgery through quasi-randomized controlled trial,relevant data were analyzed to provide a basis for the optimization of infectious wound treatment after surgery.Methods:From October 2020 to May 2023,60 patients with infectious wounds after orthopedic surgery in our hospital were divided into the experimental group and the control group,with 30 patients in each group.The control group was given VSD combined with saline irrigation,while the experimental group was given oxygen irrigation on the basis of the control group.The incision volume and granulation tissue coverage were observed and recorded by researchers and wound therapists before and 7 d after intervention;granulation tissue coverage and volume reduction rate between groups;Before and 7 days after the intervention,the wound secretion was taken for bacterial culture,and the results of bacterial culture were recorded.Then,according to the wound condition,the second stage operation was decided or the dressing was refilled(replaced)to continue the closed drainage until it healed,and the wound healing rate and time were observed.Observe whether there are any adverse reactions such as bleeding,dehiscence,infection,etc.In the whole process from wound treatment to healing.Results:1.Baseline data:In the general basic data,there were no significant differences in gender,age,BMI,nutrition,admission method and postoperative complications between the two groups(P>0.05).In the clinical indicators before wound treatment,there were no significant differences in incision length,incision volume and bacterial culture results between the two groups(P>0.05).These results indicated that the general baseline data and wound clinical indexes of the two groups were consistent and comparable.2.Clinical indicators:(1)Granulation tissue coverage rate: Granulation tissue coverage rate of each group between before and after intervention was statistically significant(P<0.05).After 7 d of intervention,the granulation tissue coverage rate of the experimental group and the control group was(89.81±6.00)% and(74.52±4.85)%,respectively,the difference was also statistically significant(P<0.05).(2)Wound volume reduction rate: Wound volume in each group between before and after intervention was statistical significance(P<0.05).After 7 d of intervention,the experimental group volume reduction rate was(53.22±3.98)%,control group was(35.04±9.70)%,the difference was also statistically significant(P<0.05)..(3)Bacterial culture results: After 7 d of intervention,the positive rate of bacteria was13.33% in the experimental group and 33.33% in the control group,with no statistical significance(P>0.05).(4)Healing rate and healing time: 27 patients in the experimental group had a healing rate of 90%,23 in the control group had a healing rate of 76.67%,not statistically significant(P>0.05);while the mean healing time was(31.23±8.77)days in the experimental group and(34.13±6.97)days in the control group,and the difference was not statistically significant(P>0.05).Conclusion:Both treatment measures can reduce the wound volume and have good safety.Compared with the vacuum sealing drainage combined with irrigation of normal saline,the vacuum sealing drainage combined with oxygen infusion irrigation treatment is more effective in promoting the growth of granulation tissue,improving the coverage of granulation tissue,which is worthy of clinical application.
