Femoroacetabular Impingement syndrome(FAI),as one of the diseases with hip pain and activity limitation as the main clinical manifestations,is mostly due to abnormal contact and impingement between the femoral head-neck junction and the acetabular rim due to abnormal covering morphology of the acetabulum and rotation of the femur,and early manifests only as activity after In the early stage,the pain is only in the inguinal region after activity,and then there are micro-injuries to the acetabular labrum and cartilage,and the joint degeneration can evolve into primary osteoarthritis in the long term.As a chronic hip disease with insidious onset,its pathological changes are not easily detected at an early stage,and the late treatment for it is limited and ineffective,and patients can only be treated with hip arthroscopy or hip replacement,which makes patients bear a great burden and reduces their postoperative living standard.Currently,the treatment of FAI is based on early conservative prevention and treatment and late surgery to correct the anatomical shape of the hip joint.This study provides a clinical treatment method for early stage FAI and investigates the possible therapeutic mechanism of this method in treating the disease.This clinical study was reviewed and approved by the Ethics Committee of the First Hospital of Heilongjiang University of Traditional Chinese Medicine(ethical review approval number: HZYLLKT202209001),and all clinical study procedures strictly complied with the Ministry of Health’s "Ethical Review Measures for Biomedical Research Involving Human Beings(2016)",CFDA’s "Quality Management Specification for Drug Clinical Trials(2020)","Medical Device Clinical Trial Quality Management Standards(2016)",WMA "Declaration of Helsinki(2013)" and CIOMS "International Ethical Guidelines for Human Biomedical Research(2002)" for ethical principles.Objective:1.To compare the clinical efficacy of extracorporeal shock wave combined with Chinese herbal fumigation with that of extracorporeal shock wave alone and Chinese herbal fumigation alone in the treatment of early FAI.2.To investigate the effect of this method on hip pain,function and patients’ quality of life level in the treatment of early FAI.3.To explore effective methods for the non-surgical treatment of early FAI and to reduce patients’ pain and suffering for clinical practice.Methods:According to the inclusion and exclusion criteria,102 patients with FAI who visited the outpatient clinic and ward of the First Department of Orthopedics of Heilongjiang University of Traditional Chinese Medicine between November 2021 and January 2023 were randomly divided into three groups: 34 patients in the Chinese herbal fumigation group,34 patients in the extracorporeal shock wave group,and 34 patients in the comprehensive treatment group,the Chinese herbal fumigation group treated with general basic treatment and Chinese herbal fumigation,the extracorporeal shock wave group treated with general basic treatment and extracorporeal shock wave treatment.And the patients in the comprehensive treatment group were given general basic treatment and combined treatment of Chinese medicine fumigation and extracorporeal shock wave treatment.The patients in the three groups were randomly divided into three groups: 34 patients in the Chinese herbal fumigation group,34 patients in the extracorporeal shock wave group and 34 patients in the comprehensive treatment group.One course of treatment was given to the three groups of patients respectively,and the changes in the Visual analogue scale(VAS),Harris hip score(HHS)and quality of life of the three groups of patients before and after treatment were recorded and counted.Results:There was no statistically significant difference in age,gender and disease duration(P>0.05),and no statistically significant difference in VAS,HHS and quality of life level(P>0.05)among the three groups of patients before treatment.After 1 course of treatment,the observed indexes before and after treatment were statistically analyzed separately for the three groups of patients,and the symptoms of patients in all three groups improved after treatment,and the changes in VAS,HHS and quality of life level of patients were significantly different from those before treatment(P<0.05).When the VAS,HHS and quality of life level scores of patients in the three groups were compared two by two after treatment,there was no statistically significant difference between the Chinese herbal fumigation group and the extracorporeal shock wave group(P>0.05).In the inter-group comparison between the comprehensive treatment group and the Chinese herbal fumigation group and the extracorporeal shock wave group,the treatment effect of the comprehensive treatment group was significantly better than that of the Chinese herbal fumigation group and the extracorporeal shock wave group,and the differences were statistically significant(P<0.05).Conclusion:The organic combination of traditional Chinese herbal fumigation and extracorporeal shock wave therapy has obvious effects on relieving the local pain of FAI,improving the functional activities of the hip joint,and improving the quality of life of patients.At the same time,it can effectively avoid the adverse reactions of oral drugs and surgery.The damage caused by it is very effective for FAI.
