| Objective The purpose of this study was to investigate the dynamic changes of left atrial function and its relationship with inflammation in patients with persistent atrial fibrillation after radiofrequency ablation using the latest 4DAuto LAQ technique.Methods100 patients with persistent atrial fibrillation scheduled for radiofrequency ablation in our Hospital from January 2021 to November 2021 were selected.The presystolic volume,maximum volume of left atrium,maximum volume index of left atrium,minimum volume of left atrium,anterior and posterior diameter of left atrium,ejection fraction of left atrium,longitudinal strain of left atrial storage and longitudinal strain of left atrial conduit were obtained by blood routine examination,routine ultrasonography and 4D Auto LAQ analysis 1 day before radiofrequency ablation and 1 day,1 week,1month and 3 months after radiofrequency ablation.Longitudinal strain of left atrial contraction,circumferential strain of left atrial storage,circumferential strain of left atrial conduit,circumferential strain index of left atrial contraction,total leukocyte count,hypersensitive reaction protein and so on.In order to analyze the dynamic change trend of left atrial function and its relationship with inflammatory indexes in patients with persistent atrial fibrillation after radiofrequency ablation.ResultExcluding 20 patients who could not continuously monitor the changes of ECG,a total of 80 patients were included in this study:(1)presystolic volume,maximum volume of left atrium,maximum volume index of left atrium,minimum volume of left atrium,anterior and posterior diameter of left atrium before radiofrequency ablation of atrial fibrillation,1 day,1 week and 1 month after radiofrequency ablation,the difference was statistically significant at 3 months after operation(P < 0.05).Compared with before operation,the maximum volume of left atrium decreased gradually in 1 day,1week,1 month and 3 months after operation.The maximum left atrial volume index,left atrial presystolic volume,left atrial anterior-posterior diameter and minimum left atrial volume increased slightly at 21 days and 1 week after operation,and then decreased slowly.There was significant difference in left atrial ejection fraction before operation,1 day,1 week,1 month and 3 months after operation(P < 0.05).It decreased on the first day,decreased significantly on the 7th day after operation,and then increased slowly(2)four-dimensional volumetric strain parameters of left atrium,including longitudinal strain parameters of left atrium and circumferential strain parameters of left atrium,were significantly different before,1 day,1 week and 1 month after radiofrequency ablation of atrial fibrillation(P < 0.05).They all decreased slightly on the first day after operation,decreased significantly 1 week after operation,and then increased slowly.(3)The postoperative inflammatory index of persistent atrial fibrillation was higher than that before operation(P < 0.05).The total number of white blood cells and the level of hypersensitivity reactive protein were significantly higher than those before operation on the 1st day and 1 week after operation(P < 0.05).After that,it decreased gradually,and the peak value appeared 1 week after operation.(4)One week after radiofrequency ablation in patients with persistent atrial fibrillation,the left atrial ejection fraction was correlated with Hs-CRP level(r = 0.43 ~ 0.035).At the same time,this study found that the change trend of left atrial strain parameters and inflammatory indexes were basically the same,which worsened briefly within 1 month after operation,and then gradually returned to the preoperative level.ConclusionThe latest 4D Auto LAQ analysis showed that there was a short period of deterioration of left atrial function in patients with persistent atrial fibrillation after radiofrequency ablation,and the changes were significantly improved about 3 months after radiofrequency ablation,and its changes were related to inflammation. |
