Objective: To analyze the clinical data of RUNX1-RUNX1T1-positive AML adult patients,and to explore the relevant factors affecting the prognosis of RUNX1-RUNX1T1-positive AML patients,so as to guide the treatment and prognosis assessment of RUNX1-RUNX1T1-positive AML patients.Methods:The clinical data of 64 patients with newly treated RUNX1-RUNX1T1-positive AML admitted to our hospital from October 2016 to December 2022 were retrospectively analyzed.Using the Kaplan-Meier method,the log-rank test analyzed the factors that affected the overall survival time(OS)and recurrence-free survival(RFS)of RUNX1-RUNX1T1 fusion gene positive patients,and the Cox regression model was used to further analyze the relevant factors affecting prognosis.Results:In the analysis of prognostic factors in AML patients with positive RUNX1-RUNX1T1 fusion gene,univariate analysis showed that CD19 positive,MRD decreased by > 3 logarithmic levels after induction therapy,HAA was selected for induction regimen,high-dose cytarabine consolidation therapy,and fusion gene conversion after treatment were factors affecting the prognosis of OS(P<0.05),and the white blood cell ≥ was 20×109/L,C-Kit D816 mutation was a factor affecting the poor prognosis of OS and RFS(P<0.05),while a decrease of 3 logarithmic levels of MRD after high-dose cytarabine consolidation therapy and >induction therapy,and the conversion of fusion genes were factors that affected the prognosis of RFS(P<0.05).The results of multivariate analysis showed that WBC count ≥20×109/L,C-Kit D816 mutation,MRD decreased by > 3 logarithmic levels after induction therapy,HAA induction therapy,and fusion gene conversion were all independent prognostic factors of OS.C-Kit D816 mutation and fusion gene conversion are independent prognostic factors of RFS.Conclusion:This study found that the prognosis of positive AML of RUNX1-RUNX1T1 fusion gene included various factors such as white blood cell count,treatment plan,C-Kit D816 mutation,and fusion gene negative,so it should be monitored in the initial clinical treatment and subsequent consolidation review.
