| Objective:To analyze whether there is a correlation between vertebral CT values and the occurrence of cement leakage in patients with osteoporotic vertebral compression fractures(OVCF)treated with percutaneous vertebroplasty(PVP).Methods:Patients who were hospitalized for osteoporotic vertebral compression fractures and treated with PVP between September 2019 and October 2022 at our hospital were collected,All patients were divided into two groups(leaking group and non-leaking group)according to whether bone cement leakage occurred or not,First find the inpatient records of eligible patients through the hospital’s new Tung Wah system.and recorded the gender, age and intraoperative volume of bone cement injected in both groups,The patient’s imaging data were then analyzed and tallied in the PACS system to record the fracture site(thoracic or lumbar),the degree of fracture compression and the CT values of the lumbar 1 vertebrae,All the collected data were divided into count data and measurement data, and according to the data characteristics of each group, the statistical software SPSS 26 was used to test whether the above data were statistically different using chisquare test and non-parametric test(P<0.05),respectively,The statistically different factors were then analyzed by binary logistic regression of the data(P<0.05),ROC curves were also plotted to further investigate the correlation between CT values of the lumbar 1 vertebral body and the o-ccurrence of bone cement leakage via vertebroplasty. Results:Of the 61 patients collected,13 were male patients and 48 were female patients with a total of 61 vertebrae,45 were thoracic compression fractures and 16 were lumbar compression fractures, the range of bone cement injected into all fractured vertebrae was between 3-8 ml,of which 39 vertebrae had bone cement leakage and 22 vertebrae had no leakage,After all data were included in the statistical analysis,it was found that there was no statistically significant difference between the two groups in terms of gender, age, fracture site(thoracic or lumbar spine)and the amount of bone cement injected(P>0.05),The differences in the degree of fracture compression and CT values of lumbar 1 vertebrae were statistically significant(P<0.05),and The difference in the change in CT values of the lumbar 1 vertebrae was statistically significant when the two were included in a binary logistic analysis(P<0.05),and further ROC curve analysis was done, and the best cutoff value for the lumbar 1 vertebral CT values was obtained at 62.1HU,with a sensitivity of 79.5%and specificity of 63.6%,and an area under the curve(AUC)of 0.674(95%CI:0.524 to 0.824,P=0.025).Conclusion:Lumbar 1 vertebral body CT values correlate with cement leakage in osteoporotic vertebral compression fractures treated with PVP,and measuring patients’ lumbar 1 vertebral body CT values preoperatively is a usef-ul guide in assessing cement leakage. |
