| Objective:1.To observe the difference in brain functional activity between SIVD patients and healthy people in resting state by rs-f MRI technique.2.To observe the changes in cognitive level of SIVD patients after the intervention of Huayu Tongluo Moxibustion and to evaluate the clinical efficacy.3.To observe the changes of local spontaneous neuronal activity in the brain of SIVD patients after the intervention of Huayu Tongluo moxibustion,and to investigate the intervention of Huayu Tongluo moxibustion on the brain functional activity of SIVD patients.Methods:Thirty patients with SIVD were recruited as the treatment group(PT group) and 30 healthy people of similar age were recruited as the control group(HC group).The PT group was treated with Huayu Tongluo Moxibustion,and the frequency of treatment was 3 times a week for a total of 24 weeks.The PT group was assessed once before treatment and once after 24 weeks of treatment with neuropsychological scales including the Brief Mental State Examination(MMSE),the Montreal Cognitive Assessment Scale(Mo CA),and the Modified Hasegawa Dementia Scale(HDS-R),and each completed a resting-state functional magnetic resonance scan.The HC group filled out the MMSE,Mo CA and HDS-R measurement scales,and patients were scanned once before treatment and once after 24 weeks of treatment for MRI.Analytical processing of scale results by application software;the MRI data were processed and analysed using the low frequency amplitude(ALFF)and ratiometric low frequency amplitude(f ALFF)methods using MATLAB R2021 a and DPABI 6.2 software.Results:1.Before treatment with Huayutongluo moxibustion,scored significantly lower on the MMSE scale compared with the HC group(P<0.01);the Mo CA(P<0.01)score was significantly lower;and scored significantly lower on the HDS-R scale compared with the HC group(P<0.01).brain areas with reduced ALFF were mainly distributed in the right superior frontal gyrus,and brain areas with enhancement were mainly distributed in the left temporal lobe,occipital lobe,and caudate nucleus;brain areas with reduced f ALFF The brain areas with reduced f ALFF are mainly located in the left perirhinal cortex,the right anterior cingulate and paracingulate gyrus,and the right middle frontal gyrus;the brain areas with enhanced f ALFF are in the left hippocampus and the right thalamus.2.Patients’ scores on the MMSE scale obviously increased after treatment relative to pre-treatment.(P<0.01);the Mo CA(P<0.05)score was higher;and patients’ scores on the HDS-R scale obviously increased after treatment relative to pre-treatment(P<0.01).Brain areas enhanced by ALFF were mainly distributed in the right parahippocampal gyrus,and those reduced were mainly distributed in the bilateral precentral gyrus;the brain areas with enhanced f ALFF were mainly in the left anterior cingulate and paracingulate gyrus;the brain areas with reduced f ALFF were mainly in the left precentral gyrus and the left inferior parietal limbic gyrus.Conclusion:1.rs-f MRI was able to reflect significant differences in functional brain activity between SIVD patients and healthy people.2.Huayu Tongluo Moxibustion may activate the functional network in certain specific regions of SIVD patients to make the values of ALFF and f ALFF tend to change benignly.3.Huayu Tongluo Moxibustion can effectively improve the cognitive dysfunction of SIVD patients,and is an effective clinical treatment for SIVD. |
