ObjectivesThe purpose of this study is to compile the competency questionnaire of infection control nurses based on the competency iceberg model,so as to provide an objective tool for accurately evaluating the competency of infection control nurses;Using this questionnaire to investigate the competency level of infection control nurses in Hunan Province and explore the influencing factors,so as to provide reference for the construction of team in nosocomial infection management.Methods(1)Formation of the competency questionnaire for infection control nurses: Based on the competency iceberg model,pool of items of the competency questionnaire for infection control nurses was formed on the basis of reviewing the relevant literature on the competency of infection control nurses in the early stage,and the primary,secondary and tertiary indicators were preliminarily drawn up;The primary,secondary and tertiary indicators of the questionnaire were preliminarily determined through the expert correspondence;Through convenient sampling,infection control nurses in Hunan Province were selected for pre-survey,items were screened through discrimination analysis and correlation analysis,and the reliability and validity were tested by exploratory factor analysis,content validity and internal consistency reliability.(2)Cross sectional survey on the competency of infection control nurses: A cross-sectional survey was conducted on 1010 infection control nurses in Hunan Province by using the self-made questionnaire,so as to understand the competency level of infection control nurses and analyze its influencing factors.Categorical variables were described by frequency and percentage(%),and continuous variables were expressed by mean±standard deviation((?)±s)or median and quartile.Mann-WhitneyU test and Kruskal-Wallis H test were used for univariate analysis,and multivariate linear regression was used for multivariate analysis to explore the influencing factors of competency of infection control nurses.Results(1)Formation of the competency questionnaire for infection control nurses:(1)Literature research results: According to the literature retrieval strategy,27 literatures related to the research theme were included.Six primary indicators,17 secondary indicators and 92 tertiary indicators were preliminarily formulated.(2)Results of expert correspondence: The recovery rate and effectiveness rate of the first round of expert correspondence were 100%,the expert authority coefficient is 0.93,the index CV of 98.91% was less than 0.25,and Kendall’s W was 0.20.The recovery rate and effectiveness rate of the second round of expert correspondence were 100%,the expert authority coefficient was 0.93,the index CV of 97.96% was less than 0.25,and Kendall’s W was 0.24.The initial questionnaire on the competency of infection control nurses was formed,including 6 primary indicators,18 secondary indicators and 86 tertiary indicators.(3)Pre-investigation results: After item analysis,the discrimination analysis results of all items were statistically significant(P<0.05).The correlation coefficient between the total score of each item and the corresponding dimension,the total score of each dimension and the total score of the questionnaire>0.40,and there were no deleted items.Through exploratory factor analysis,3 factors were extracted from the knowledge of nosocomial infection prevention and control,and the cumulative variance contribution rate was 75.01%;4 factors were extracted from the skills of nosocomial infection prevention and control,and the cumulative variance contribution rate was 78.79%;2 factors were extracted from the management cooperation ability,and the cumulative variance contribution rate was 73.54;2 factors were extracted from teaching and research ability,and the cumulative variance contribution rate was 78.43%;2 factors were extracted from personal characteristic quality,and the cumulative variance contribution rate was 82.38%;1factor was extracted from the motivation,and the cumulative variance contribution rate was 68.78%.The final questionnaire has 6 primary indicators,14 secondary indicators and 69 tertiary indicators.The total content validity of the questionnaire is 0.93,the content validity of each item is 0.84~1.00,and the Cronbach’s α of the questionnaire is 0.983,the Cronbach’s α of 6 primary indicators is 0.92 ~ 0.97,the split half reliability of the questionnaire is 0.89.(2)Cross sectional survey on the competency of infection control nurses: The average score of the total items was(3.82±0.53).The average scores of 6 primary indicators from high to low was personal characteristic quality(4.22±0.50),knowledge of nosocomial infection prevention and control(3.96±0.54),motivation(3.89±0.65),management coordination ability(3.85±0.56),nosocomial infection prevention and control skills(3.75±0.64),teaching and research ability(3.47±0.66).Through multiple linear regression analysis,it can be seen that working year,technical title,average monthly income,highest degree,hospital grade,whether it is a teaching hospital,position and job category were included in the regression equation(P<0.05).Conclusions(1)This study compiled the competency questionnaire for infection control nurses,which contains 6 primary indicators,14 secondary indicators and 69 tertiary indicators.It has good reliability and validity and can objectively reflect the competency level of infection control nurses.(2)Competency of infection control nurses in Hunan Province is at the upper-middle level.Among 6 primary indicators,the scores of personal characteristics,motivation and management,coordination ability and knowledge of nosocomial infection prevention and control are higher than the average score of all items,and the skills of nosocomial infection prevention and control,teaching and research ability were lower than the average score of all items.Therefore,the focus of improving the competency of infection control nurses in the future is to improve their teaching and research ability and strengthen the training of nosocomial infection prevention and control skills including nosocomial infection detection,risk identification and information construction,etc.(3)The influencing factors of competency of infection control nurses are working year,technical title,average monthly income,highest degree,hospital grade,whether it is a teaching hospital,position and job category.
