Objective:To investigate the correlation between cerebrospinal fluid(CSF)glial fibrillary acid protein(GFAP)level and the core markers and cognitive scores of Alzheimer’s disease(AD),as well as its association with the progression of AD,the application value of CSF GFAP in AD disease was analyzed.Methods:The study included cognitively normal controls with CSF GFAP data from the Alzheimer’s Disease Neuroimaging Initiative(ADNI)(CN,n=86),mild cognitive impairment(MCI,n=134),and AD(n=65).Each group was divided into different subgroups according to tau pathological status and gender.Nonparametric test was used to compare CSF GFAP levels between groups.Spearman correlation analysis was used to determine the correlation between CSF GFAP level and AD core biomarkers and cognitive scores.The predictive value of CSF and GFAP in AD progression was analyzed by receiver operating curve(ROC).K-M survival analysis assessed the association of CSF biomarker levels with disease progression.Results:1.The results of inter-group comparison showed that the level of CSF GFAP in AD group was higher than that in CN group;The CSF GFAP level of tau+in CN and MCI groups was higher than that of tauin different tau pathologic subgroups.The CSF GFAP level of men in AD group was higher than that of women in AD group in different clinical subgroups.2.Spearman correlation analysis showed that CSF GFAP level was positively correlated with age in all diagnostic groups,but not with years of education;There was no correlation with CSF Aβ level,but positive correlation with CSF t-tau level.In CN and MCI groups,CSF GFAP levels were positively correlated with CSF p-tau levels.CSF GFAP level was negatively correlated with MMSE score and positively correlated with CDRSB score in each diagnostic group.3.CSF GFAP level was a good predictor of the progression risk of CN to MCI,and the value of AUC was 0.79.4.CN population and MCI population with high CSF GFAP level are more likely to develop MCI and AD.Conclusions:1.CSF GFAP level is significantly correlated with tau pathology and cognitive decline in AD,which is of great value for the diagnosis and progression of AD and can be used as an effective supplement for existing biomarkers of AD.
