Background:Mild Cognitive Impairment(MCI)is the transitional stage between cognitive normality and dementia,and is a suitable and necessary stage for the prevention and early treatment of dementia.Studies have shown that computerized cognitive training can improve cognitive and psychosocial function in people with MCI.As a kind of computerized cognitive training,virtual supermarket is based on virtual reality technology to create virtual shopping scenes,which have the characteristics of immersion,interaction and conception,which can greatly break through time and space limitations and realize functional training in multiple cognitive fields of MCI population.Most of the existing studies focus on the use of virtual supermarkets to evaluate and screen people such as MCI and dementia,and some studies have shown that it can improve the cognitive function of elderly people with MCI.However,at present,there is no virtual supermarket intervention study involving multi-domain cognitive training for the elderly with MCI in China.Objectives:1.To explore the intervention effect of the virtual supermarket cognitive assessment and training system involving multiple cognitive fields independently developed by the research group on cognitive function,depression,anxiety level and quality of life in the elderly with MCI.2.To understand the feelings and experiences of the research subjects participating in cognitive training in a virtual supermarket,analyze the influencing factors of the intervention,and lay the foundation for further improving and promoting virtual supermarket cognitive training.Methods:1.Intervention effect of virtual supermarket cognitive training on the elderly with mild cognitive impairment: this study is a randomized controlled trial conducted in a nursing home in Changchun City from November 2021 to January 2022.Forty-two elderly people with MCI were selected and divided into virtual supermarket group and control group,with 21 people in each group.The virtual supermarket group used virtual supermarket cognitive training for 30 minutes twice a week for a total of 6 weeks,and the control group maintained their daily lifestyle.Primary outcomes included cognitive function,and secondary outcomes included depression,anxiety,quality of life,and system availability.Assessment was assessed at baseline and six weeks after the intervention.Assessment tools include the subjective questionnaires,the Mini-Mental State Examination(MMSE),the Montreal Cognitive Assessment(Mo CA),the Geriatric Depression Scale(GDS-15),the Geriatric Anxiety Inventory(GAI-20),the Short-Form Health Survey(SF-12)and the System Usability Scale(SUS).SPSS 23.0software is used for data analysis.2.A qualitative study on the cognitive training experience of virtual supermarkets in the elderly with mild cognitive impairment: using descriptive qualitative study,through the purpose sampling method,the elderly who have completed 6 weeks of virtual supermarket cognitive training were selected as interview subjects during September 2022 ~ October 2022 for one-on-one and semi-structured interviews.Using the content analysis method,the qualitative analysis software Nvivo11 was used to organize and analyze the interview data.Results:1.The intervention effect of virtual supermarket cognitive training on the elderly with mild cognitive impairment: there was no significant difference between the two groups in comparison of general demographic data and baseline data(p > 0.05).After6 weeks of intervention,compared with the control group,the MMSE score,Mo CA score,global cognition score and quality of life score of the elderly in the virtual supermarket group were significantly improved,and the score of depressive symptoms was significantly reduced.Compared with the baseline,the scores of anxiety symptoms in the virtual supermarket group were significantly reduced,while the difference in the control group was not statistically significant.The total score of the participants’ evaluation of the usability of the virtual supermarket system is greater than 60 points,indicating that the usability of the virtual supermarket system is good and the user experience is good.2.A qualitative study of cognitive training experiences in virtual supermarkets in older adults with mild cognitive impairment: the interview data were analyzed and refined to form four main categories: "Perception of participation in activities","Benefits related to activities","Motivation factors for persistence activities" and "Hindrance factors for persistence activities".Perceptions of participating in activities include well-designed activities,improved brain function through participation,emotional satisfaction from participating in activities,and win-win outcomes for health promotion and scientific research.Activity-related benefits include improved individual cognitive function,mood regulation,social interaction,increased learning ability,and self-actualization.Participation in activities can make participants feel socially cared for and happy,and emotional improvement can also allow patients to participate in activities with a better emotional state and mental outlook,which may help improve the effectiveness of activities.Activity characteristics,sense of achievement,patient guidance of staff,young and energetic,cooperation with the activity are the promotion factors for participants to persist in participating in the activity,age,physical discomfort,competition priority,staff is not fixed,and some participants believe that the operation has a certain difficulty are the hindrance factors to participating in the activity.Conclusions:1.The virtual supermarket cognitive assessment and training system independently developed by the research group involves multiple cognitive fields,and the training function is comprehensive,rich in content and has good availability and acceptability;2.Virtual supermarket cognitive training can improve the global cognitive,depressive symptoms,anxiety symptoms,and quality of life of the elderly with MCI;3.Participants subjectively felt that participating in activities improved individual cognitive function,regulated emotions,promoted social interaction and improved learning ability.In the process of future promotion,promoting factors should be increased,obstructive factors should be reduced or avoided,and the implementation of intervention programs should be improved.
