| Objective:By analyzing the data related to fibrin glue-assisted retrograde intrarenal surgery(FGaRIRS)and standard retrograde intrarenal surgery(StRIRS)performed by the same urological surgeon during the same period,to evaluate the efficacy and safety of FGaRIRS in terms of the stone clearance rate(SCR),stone free rate(SFR)and complications.Methods:Clinical data of patients diagnosed with upper urinary tract calculi and successfully performed FGaRIRS or STRIRS by the same urological surgeon in the First Affiliated Hospital of Nanchang University from June 2021 to December 2021were retrospectively collected.Statistical methods were used to compare and analyze the relevant situation of stone clearance rate,stone free rate and complications on the first day and the first month after surgery.SCR was calculated as (1-(residual stone volume)/(preoperative stone volume))×100%and SFR was calculated as ((No.of stone free patients)/(No.of total patients))×100%.Results:100 patients receiving StRIRS(n=50)or FGaRIRS(n=50)were enrolled between June 2021 and December 2021.There were no significant differences in age,BMI,gender,maximum stone diameter,stone volume,hydronephrosis and other demographic characteristics and preoperative indexes between the two groups(P>0.05).Patients in the FGaRIRS group received a mean of 2.7±0.8 cycles of gel injection and extraction.The SCR in the FGaRIRS group were significantly higher than those in the StRIRS group on the first day and the first month after surgery(99.1%vs 68.2%,P<0.001;99.6%vs 81.6%,P<0.001).On the first day after surgery,26patients in the FGaRIRS group had no residual stone,SFR was 52%,while all patients in the StRIRS group had residual stone.At the first month after surgery,35patients in the FGaRIRS group were completely free of stones,SFR was 70%,while StRIRS was 0.None of the patients had blood transfusions,perforations,or hematoma in both groups.There was no postoperative fever in the StRIRS group while 1 patient in the FGaRIRS group.8 patients had postoperative infection in the StRIRS group while 4 patients in the FGaRIRS group.The complication rates of the two groups were comparable(P>0.05).Analysis of stone composition showed no statistical significance between the two groups(calcium oxalate,phosphate,uric acid and struvite with 27 vs 25,8 vs 9,8 vs 10 and 7 vs 6,P=0.933).Conclusion:Compared with StRIRS,FGaRIRS have higher stone clearance rate and stone free rate.FGaRIRS is an effective and safe surgical technique,which has the value of further application.Larger sample sizes and multi-center studies are needed for further evaluation. |
