ObjectiveTo verify the safety and effectiveness of ultrasound-guided transabdominal radiofrequency ablation for the treatment of uterine leiomyoma based on the preoperative and postoperative myoma symptoms and health quality of life scores as well as intraoperative and postoperative complications.We also analyzed the curative effect of radiofrequency ablation on symptomatic uterine macromyoma after operation,and discussed the influencing factors of short-term curative effect of radiofrequency ablation of uterine macromyoma,so as to provide reference for clinical evaluation of short-term curative effect of radiofrequency ablation.Materials and methods52 fibroids from 45 patients who underwent ultrasound-guided radiofrequency ablation of abdominal fibroids in the Affiliated Hospital of Yan’an University from December 2020 to September 2022 were retrospectively collected as the study subjects.Before and after the operation,the patients filled in and scored the hysteromyoma symptoms and healthy quality of life questionnaire.The paired sample t-test was used to analyze whether there was significant difference in the preoperative and postoperative hysteromyoma symptoms and healthy quality of life questionnaire scores,Judge the effectiveness of ablation treatment;The intraoperative and postoperative complications were recorded to verify the safety of ablation treatment.According to the effect of three months after ablation,the patients were divided into the excellent treatment group(myoma reduction rate ≥ 50%)and the good treatment group(myoma reduction rate<50%).The general clinical data,ultrasound-related indicators and some laboratory indicators between the two groups were analyzed and compared.The independent influencing factors of the effect of radiofrequency ablation of uterine fibroids were screened by single factor and binary logistic regression analysis,and the receiver operating characteristic(ROC)curve was drawn,Compare the prediction probability value of regression model and the prediction efficiency of each independent influencing factor.ResultsThere were statistically significant differences in myoma-related Assess the symptom status(SSS),health-related quality of life(HRQOL)scores and hemoglobin levels of patients with hysteromyoma before and after ultrasound-guided transabdominal radiofrequency ablation treatment of hysteromyoma(t=17.2,P <0.000,t=-15.6,P <0.000,t=-11.1,P <0.000);There were no major complications during and after operation.The univariate analysis of the effect of postoperative treatment showed that there were differences between the excellent group and the good group(X~2= 4.038,P=0.044,X~2= 12.239,P<0.000,X~2= 5.042,P=0.025,X~2= 8.421,P=0.004,X~2= 7.181,P=0.007)。 The binary logistic regression analysis found that the size of myoma,blood flow grade,whether it is uniformly enhanced,and the energy received per unit volume of myoma were independent influencing factors of short-term efficacy(P<0.05).The ROC curve analysis showed that the area under the curve(AUC)predicted by the logistic regression model was0.956,the sensitivity was 0.857,the specificity was 0.947,and the AUC was significantly higher than other independent factors.Conclusions(1)Ultrasound-guided transabdominal radiofrequency ablation is safe and effective in the treatment of big uterine leiomyoma.(2)The myoma‘ blood flow grade,homogeneous enhancement,and the energy received per unit volume of myoma are independent factors affecting the short-term efficacy of ultrasound-guided transabdominal radiofrequency ablation for the treatment of uterine leiomyoma...
