Abstract/Summary:
ObjectiveTo explore the association between carotid and coronary artery stenosis severity in patients with UA and / or without t2 dm risk factors using observational analysis of carotid and coronary artery stenosis severity in patients with unstable angina pectoris(UA)and non type 2 diabetes mellitus(T2DM),Thus,we can provide a theoretical basis for the early detection of UA atherosclerosis by this noninvasive means of carotid ultrasound in clinic,and can predict patient prognosis based on peripheral vascular lesions,and provide a theoretical basis for the further diagnosis and treatment of patients with coronary artery disease.MethodsOne hundred and twenty consecutive patients who were admitted to the Department of Cardiology,the Second Affiliated Hospital of Shenyang Medical College,with a confirmed diagnosis of UA,and underwent coronary angiography(CAG)and carotid ultrasonography from December 1,2021 to December 31,2022,were enrolled,and the basic data of the patients were collected.Coronary angiography: right radial artery puncture by the epidermis,if necessary to obtain(right brachial artery,right femoral artery),push the contrast catheter to reach the coronary opening,push the contrast agent under the X-ray view the coronary development,to view the vessel in a multiple body position(left anterior oblique,right anterior oblique,head position,foot position,etc.),after the operation,the patient was instructed to puncture the limb to extend the brake for 6 hours,pay attention to the situation of bleeding at the puncture site,and pay attention to the situation of the peripheral circulation.The patients were classified according to the quantitative assessment criteria for the degree of coronary stenosis by Gensini scoring system according to the segmented criteria for scoring coronary vascular images developed by the American Heart Association as follows: 41 in the low group(1-30 points),29 in the middle group(31-60 points),and 50 in the high group(> 60 points).Carotid intima media thickness(IMT)and peak velocity(PSV)were compared among the three groupsAll enrolled cases were examined by color Doppler sonography by two skilled and qualified CEUS Color ultrasound examination was performed by the room physician.Subjects were positioned in the occipital supine position with the shoulder pads high head back to sufficiently expose the neck,The head is biased to the contralateral side of the examined side,and the probe is moved stepwise upward from the supraclavicular fossa to probe the common and internal carotid arteries,respectively And external carotid artery.Data such as carotid intima-media thickness(IMT)and peak velocity(PSV)measured by carotid ultrasound were recorded.A further 60 patients with T2 DM were included as an observation group,and 60 UA patients without T2 DM were included as a control group to compare the differences between the two groups in baseline demographic characteristics,coronary Gensini scores,and so on.All data were processed using SPSS 29.0,and the measurement data were presented as(x ± s)for statistical description,and all the quantitative data items(IMT,PSV,etc.)that met the normal distribution were analyzed by t-test for statistical difference,and linear correlation analysis was performed using simple linear regression for quantitative data of two groups,with P < 0.05 as significant difference.ResultsIn this study,120 UA patients who underwent coronary angiography were prospectively analyzed,and baseline data such as age,gender,lipid profile were statistically analyzed between 60 T2 DM patients with UA and 60 UA patients without T2DM(P > 0.05).By analyzing the data among groups,IMT and PSV were compared among low,medium,and high subsets of coronary Gensini score,and significant differences were found between medium,high,and low subsets of IMT and PSV(P <0.05),and between high and medium subsets of IMT and PSV(P < 0.05).There were significant correlations between Gensini scores and both IMT and PSV,with R2 values of 0.803,0.695,respectively,P < 0.01 in both groups.Significant difference in Gensini score between T2 DM patients with UA and non-t2 dm patients with UA,* P < 0.05.Conclusions1.1.The carotid IMT and PSV can reflect the degree of coronary stenosis,and have a positive correlation,which has a predictive effect on the degree of coronary stenosis.2.Diabetes mellitus is an independent influencing factor for the occurrence and development of coronary stenosis,and diabetes mellitus has a greater influence than non diabetes mellitus.
