Objective:To investigate the expression of the protein encoded by phosphatase and tensin homology deleted on chromosome ten and E-cadherin in middle ear cholesteatoma,and to analyze the role and correlation of the two in the middle ear cholesteatoma,in order to find a theoretical basis for non-surgical gene therapy for middle ear cholesteatoma.Methods:1.Specimen collecting and handling:From April 2021 to January 2022,21 cases of middle ear cholesteatoma patients who underwent surgical treatment in Zhuhai People’s Hospital were selected as the research subjects.Collecting the middle ear cholesteatoma and normal skin behind ipsilateral ear,which were divided into 2 parts.One part was cured by 4%paraformaldehyde for 24h and prepared into paraffin blocks,which was detected by immunohistochemistry.The other part was stored in the refrigerator at-80℃,which was detected by Western blot.2.Experimental methods:2.1 Assessment of ear bone destruction:Graded and recorded according to the CT of temporal bone and intraoperative observation of ear bone destruction.2.2 Immunohistochemistry:A total of 21 middle ear cholesteatoma specimens and 14 behind-the-ear normal skin samples were collected.Immunohistochemistry was used to detect PTEN protein and E-cadherin in the two groups of samples.The expression differences were compared and correlation analysis was performed.2.3 Western Blot:A total of 21 middle ear cholesteatoma specimens and 18 normal skin behind the ear specimens were collected.Western Blot was used to detect the expression of PTEN protein and E-cadherin in the two groups,and the expression differences were compared and the correlation analysis was conducted.2.4 Correlation analysis was conducted between the expression of PTEN protein and E-cadherin in middle ear cholesteatoma and the destruction of ear bone in patients.Results:1.Immunohistochemistry:The average optical density value of PTEN protein and E-cadherin in middle ear cholesteatoma were significantly lower than those in normal behind-ear skin(P<0.05).There was a positive correlation between the expression of the two in middle ear cholesteatoma(r=0.711).2.Western Blot:The relative gray value of PTEN protein and E-cadherin in middle ear choleliatoma were significantly lower than those in normal behind-ear skin(P<0.05).There was a positive correlation between the expression of the two in middle ear cholesteatoma(r=0.725).3.Correlation between PTEN protein,E-cadherin and the degree of ear bone destruction:Immunohistochemistry and Western Blot analysis showed that PTEN protein and E-cadherin were significantly negatively correlated with the degree of ear bone destruction(r of PTEN protein was-0.728 and-0.868,r of E-cadherin was-0.882 and-0.798,respectively).The average optical density and relative gray value of ear bone destruction in grade Ⅰ,Ⅱand Ⅲ groups were decreased successively,and there were significant differences among groups(P<0.05).Conclusions:1.The expression of PTEN protein and E-cadherin in middle ear cholesteatoma showed significant positive correlation,suggesting that they may have mutually promoting relationship in middle ear cholesteatoma.2.In middle ear cholesteatoma,the expression of PTEN protein and E-cadherin was significantly negative associated with the damage degree of ear bone,suggesting that they may play an inhibitory role in the occurrence and development of middle ear cholesteatoma.
