Abstract/Summary:
Objective: To explore the clinical effect of cryoablation combined with endocrine therapy in early localized prostate cancer. Methods:A retrospective analysis of the clinical data of 44 Chinese patients with early localized prostate cancer in our hospital from January 1,2015 to January 1,2020,were treated with cryoablation combined with endocrine group(combined treatment group n= 24)Or endocrine therapy group alone(endocrine therapy group n=20)for treatment, and there is no significant difference in general information between the two groups(P<0.05),which is comparable.By comparing the PSA changes after treatment, the survival rate of the two groups, and the bone metastasis rate,it is finally determined whether there is a difference between the ablation combined endocrine group and the endocrine therapy group.Results:1.The follow-up time was 21 to 60 months, the combined group averaged(36.88±12.27)months;the endocrine group averaged(40.15±12.76)months,the combined group had a 1 year,3 years, and 5years survival rate of 100%,95.83%,91.67%,the 1 year,3 years, and 5years survival rate of the endocrine group were 100%,95.00%,and 60%.There was no statistically significant difference in 1year and 3years survival rates between the two groups(P>0.05),and the difference in 5 years survival rates was statistically significant(P<0.05).2.After receiving cryoablation combined with endocrine therapy and endocrine therapy alone for 3years,8.33% of patients in the cryoablation combined with endocrine therapy had bone metastases,20.00% of patients in the endocrine therapy group had bone metastases, and the two groups had bone metastases within 3 years. The difference in metastasis was not statistically significant(P>0.05).After 5 years of treatment,20.83% of patients in the cryoablation combined with endocrine therapy group had bone metastases, and 50.00% of patients in the endocrine therapy group had bone metastases. There was a statistically significant difference in bone metastasis between the two groups at 5 years(P<0.05).3.Within 2 years of receiving cryoablation combined with endocrine therapy and endocrine therapy alone, and 1month after cryoablation combined with endocrine therapy, the average PSA was(4.04±0.19)ng/ml,and after 1 month in the endocrine therapy group, the average PSA The value was(7.52±0.21)ng/ml,the difference was statistically significant(P<0.05);after 3 months of cryoablation combined with endocrine therapy, the average PSA was(3.18±0.14)ng/ml,and the endocrine therapy group was 3Months later, the average PSA was(4.44±0.16)ng/ml,the difference was statistically significant(P<0.05);after 18 months of cryoablation combined with endocrine therapy, the average PSA was(2.82±0.17)ng/ml After 18 months in the endocrine therapy group, the average PSA(10.94±0.49)ng/ml difference was statistically significant(P<0.05);after 24 months of cryoablation combined with endocrine therapy, the average PSA was(11.25±0.59)ng/ml.After 24 months in the endocrine therapy group, the average PSA was(18.99±0.49)ng/ml,the difference was statistically significant(P<0.05).Conclusions:The prognostic effect of cryoablation combined with endocrine therapy in patients with early localized prostate cancer is higher than that of endocrine therapy alone.Cryoablation combined with endocrine therapy can improve the overall survival rate of patients and reduce the rate of bone metastasis in patients with early localized prostate cancer. This study is a retrospective study. As the patient’s condition progresses, the treatment plan may change afterwards, and other treatment options such as combined surgery and targeted therapy may be selected. These treatment plans will affect the survival and prognosis of the patient to a certain extent. However, the relevant data collected so far is still insufficient,so the sub-influencing factors are not included in the analysis. In addition, due to the small number of cases in this study and the limited follow-up time, there is still a large sample of evidence to determine that combination therapy can alleviate disease progression and improve patient prognosis.
