Effect Of Inflatable Heating Blanket On Postoperative Immune Function Of Elderly Patients Undergoing Laparoscopic Radical Resection Of Colon Cancer

Objectives This study investigated the effect of inflatable heating blanket on patients’core temperature,postoperative immune function and postoperative recovery during laparoscopic colon cancer operation in the elderly,and explored the better temperature of insulation.Methods From September 2020 to September 2021,90 elderly patients(ASAⅡ-Ⅲ)who underwent elective laparoscopic radical resection of colon cancer under general anesthesia in Tangshan Workers’Hospital were selected.According to random number table method,the patients were divided into 3 groups,30 cases in each group.The three groups were group C(control group),group L(32℃gear group)and group M(38℃gear group).From the time before anesthesia induction,the control group was covered with sterile surgical sheet during the whole operation.Inflatable heating blankets were used to keep the patients warm at low temperature gear of 32℃and middle temperature gear of 38℃in group L and group M,respectively.A total of seven time points were set in this study,namely before anesthesia induction(t0),1 hour into operation(t1),2 hours into operation(t2),at the end of anesthesia(t3),1 day after operation(t4),3 days after operation(t5)and 7 days after operation(t6).We will record the mean arterial pressure(MAP),heart rate(HR),and nasopharyngeal temperature of the patients in each group at t0,t1,t2,and t3,and draw the patient’s venous blood at t0,t4,t5,and t6.This study needs to detect the content of IL-17,TGF-β,CD4+T cells,CD8+T cells in venous blood,and calculate the ratio of CD4+/CD8+.The patients were observed and recorded after operation:extubation time,whether shivering reaction occurred,whether hypothermia occurred and incision infection occurred,the time of first operative exhaust,and the length of stay after operation.Results 1 At t1,t2,and t3,the body temperature of the three groups of patients was significantly different(P<0.05).The body temperature of the three groups of patients was compared in pairs,and the body temperature of the group L was higher than that of the control group at each time(P<0.05),the body temperature in the group M was higher than that in the group L(P<0.05).2 Comparing the three groups of patients,it was found that IL-17 had no statistical significance at any time point(P>0.05).There was a statistically significant difference in TGF-βbetween the three groups at 1 day after operation(P<0.05)and at 7 days after operation(P<0.05).The comparison between the two groups showed that the TGF-βcontent of the group L and the group M was lower than that of the control group at 1 day after operation(P<0.05).At 7 days after operation,the content of TGF-βin the group M was lower than that in the control group(P<0.05).3 By comparing the three groups,the CD4+T cells,CD8+T cells,and the ratio of CD4+/CD8+were statistically significant at 1 day after operation(P<0.05)and at 3 days after operation(P<0.05).It was found by pairwise comparison between groups that the proportion of CD4+T cells in the group M was higher than that in the control group and the group L at 1 day after operation(P<0.05).The proportion of CD4+T cells in the group M and the group L was higher than that in the control group at 3 days after operation.The proportion of CD8+T cells in the group M and the group L was lower than that in the control group at 1 day after operation(P<0.05).The proportion of CD8+T cells in the group M was lower than that in the control group at 3 days after operation(P<0.05).At 1 day and 3 days after operation,the ratio of CD4+/CD8+in the group L was higher than that in the control group(P<0.05),and the ratio in the group M was higher than that in the group L(P<0.05).4 There were significant differences among the three groups in"whether hypothermia","whether shivering","extubation time",and"the time of first postoperative exhaust"(P<0.05).Pairwise comparison between groups showed that the number of people with hypothermia and shivering in the group L and the group M was less than that in the control group(P<0.05),and the number of people in the group M was less than that in the group L(P<0.05).The extubation time and the time of first postoperative exhaust in the group L and the group M were shorter than those in the control group(P<0.05),and the time in the group M was shorter than that in the group L(P<0.05).Conclusions 1 The use of inflatable heating blanket can reduce the incidence of perioperative hypothermia and shivering,and shorten the extubation time and the time of first postoperative exhaust.The effect of 38℃gear is better than 32℃gear.2 The use of inflatable heating blankets can alleviate immunosuppression in patients and may also prevent tumor recurrence and metastasis in patients.The effect of 38℃gear is better than32℃gear.Figure6;Table7;Reference 104...
