The Influence Of Family Intergenerational Solidarity On Frailty Of The Middle-Aged And Elderly People In Tangshan City

Objectives To understand the current situation of family intergenerational solidarity and frailty in the middle-aged and elderly people,analyze the influencing factors of frailty,and discuss the influence of family intergenerational solidarity on frailty of the middle-aged and elderly people,so as to provide theoretical basis for improving the frailty level of the middleaged and elderly people in the later period.Methods Stratified random cluster sampling method was used in this study.Firstly,Lubei District was randomly selected by using digital lottery method among seven administrative districts in Tangshan city,Hebei Province,then two streets were also randomly selected by using digital lottery method from eleven streets in this district,and one community was selected from each of these two streets by again using digital lottery method.In October2020 to September 2021 in the community during physical examination met inclusion and exclusion criteria,727 middle-aged and elderly people were investigated by applying General Situation Survey Scale,Family Intergenerational Solidarity Questionnaire,Comprehensive Frailty Assessment Instrument,Daily Living Ability Scale,Connor Davidson Resilience Scale,Perceived Social Support Scale face-to-face questionnaire survey.SPSS25.0 was used for statistical analysis.Descriptive method was used for descriptive the basic data,the level of family intergenerational solidarity and frailty status quo of the research object.Enumeration data were expressed as frequency and percentage,measurement data were expressed by mean and standard deviation;t-test and ANOVA were used to analyze the influencing factors of frailty in the middle-aged and elderly people.Pearson correlation analysis was used to analyze the relationship between familiy intergenerational solidarity and frailty,the stratified linear regression was used to analyze the relationship family intergenerational solidarity and frailty in the middle-aged and elderly people.P<0.05 was considered as statistically significant difference.Results 1 The scores of various dimensions in family intergenerational solidarity was as follows: the score of solidarity S dimension was(4.87±1.10),the score of offer help dimension was(2.02±0.89),the score of accept help dimension was(4.70±1.53),the score of solidarity A dimension was(3.94±0.82),and the score of normative solidarity dimension was(3.54±1.20).According to the percentage-scale conversion principle,the score of each dimension was from high to low: solidarity S,normative solidarity,accept help,solidarity A and offer help.2 The total score of frailty in the middle-aged and elderly people was(49.12±12.29),which was in the middle level.The score of each dimension was as follows:the score of physical frailty dimension was(7.52±2.37),the score of metal frailty dimension was(18.20±5.50),the score of social frailty dimension was(14.88±3.22)and the score of environmental frailty dimension was(8.52±4.66).The results of univariate analysis showed that age,marital status,educational level,personal monthly income,medical expenses,job nature,self-assessed health status,number of chronic diseases,ADL,type of character,resilience,depression,gender of children and perceived social support scale were statistically significant(P<0.05).Pearson correlation analysis showed that solidarity S,accept help,solidarity A and normative solidarity were significantly negatively correlated with frailty(r=﹣0.323、﹣0.402,﹣0.479、﹣0.371,P<0.001);the results of stratified linear regression showed that the statistically significant factors in the univariate analysis of frailty of the middle-aged and elderly people were included into the independent variables in the first layer,which could explain 20.1% variation of frailty of the middle-aged and elderly people.In the secend layer,the score of solidarity S,accept help,solidarity A and normative solidarity were explain 47.4% of frailty of the middle-aged and elderly people.Solidarity S,accept help,solidarity A,normative solidarity could independently explain 27.3%of the total variation of frailty of the middle-aged and elderly people,among them,marital status,physical labor,depression and solidarity S,accept help,solidarity A,normative solidarity were the main influence factors of frailty of the middle-aged and elderly people.Conclusions 1 The frailty score of the middle-aged and elderly people was in the middle level,which the consequences of the social frailty was striking.2 Solidarity S,accept help,solidarity A and normative solidarity were significantly negatively correlated with frailty in the family intergenerational solidarity dimension,the level of solidarity S,accept help,solidarity A and normative solidarity were high,the process of frailty was delayed and the quality of life was improved.3 Multivariate analysis results showed: the middle aged and elderly people who were widowed or otherwise,had higher depression level and had the lower socre of solidatity S,accept help,solidarity A,normative solidarity and whose previous form of labour was physical labour had hihger frailty
